Objective:The purpose of this study was to explore the relationship between quality of life and positive psychological characteristics in patients with permanent colostomy, investigate total status of the quality of life and provide the theoretical foundation for making effective psychological intervention measures.Methods:A convenience sample of 164 patients with Miles rectal cancer surgery and permanent colostomy was enrolled from three third-level grade-A hospitals in Jinan, Shandong Province and carried on questionnaire survey. The questionnaires included General information questionnaire, European Organization for Research and Treatment of Cancer Quality-of-Life Questionnaire-C30 (EORTC QLQ-C30), Connor-Davidson Resilience Scale 10 (CD-RISC 10), Herth Hope Index (HHI), the Positive and Negative Affect Scale (PANAS), which were used to collect the patients’ physiological and psychological information and evaluate the status of the quality of life, resilience, hope trait and positive affect. A total of 196 questionnaires were distributed, and 164 valid questionnaires were reclaimed, the valid return rate was 83.7%. Descriptive statistics analysis, independent sample t-test, analysis of variance, correlation analysis and multiple linear regression analysis were conducted by using the SPSS21.0.Results:1. General demographic characteristics and clinical data of participantsThe age of participants ranged from 26 to 78, with an average score of (57.54±9.86). 59.1% of cases were male.62.8% had a middle school degree or below.98.8% were married.73.2% were demission or retired.46.3% had medium income.65.9% had operation less than 6 months.32.3% with stoma complication.37.8% were against the colostomy.65.2% were cared by spouse.2. QOL score and the differences in general demographic characteristics and clinical dataThe average score of QOL in patients with permanent colostomy was (55.39±20.37). Cognitive function had the highest score (76.02±22.40) while society function had the lowest score (59.76±26.56) in function dimension; financial difficulty had the highest score (39.23±30.88) while nausea and vomiting had the lowest score (10.26± 16.55) in symptom dimension. The survey also showed that the QOL were significant differences on ages, working conditions, monthly income, operation time, colostomy complications, colostomy acceptance and caregivers.3. Positive psychological Characters score and relevance to the quality of lifeThe average scores of Resilience was (26.22±7.33), the average scores of hope trait was (21.21±3.80), the average scores of positive affect was (29.37±7.44). The Pearson correlation analysis showed that there was a positive significant correlation between all function dimension and resilience (r=0.190~0.419, P<0.05~0.001), and hope trait (r=0.293~0.430, P<0.001) except physical function dimension and general health. There was also a positive significant correlation between all function dimension and positive affect (r=0.160±0.424,P<0.05~0.001).4. Multivariate linear regression analysis of QOLAll function dimension and the general health of QOL as dependent variables, the psychological resilience, hope trait and positive affect as the independent variables were respectively included in the multiple regression analysis in the control of General demographic characteristics and clinical data. Results showed that there was a positive significant correlation between hope trait and general health (β=0.358, P=0.001), positive affect and emotion function (β=0.201, P=0.022),positive affect and general health (β=0.139, P=0.033).Conclusions:1. QOL of patients with permanent colostomy had the highest score in cognitive function, lowest score in social function; in symptom function, economic difficulties had the highest score, while nausea and vomiting had the lowest score.2. There was significant positive correlation between QOL and resilience, hope trait, positive affect.3. Hope trait and positive affect might predict the QOL of patients with permanent colostomy.
