Objective: To compare the therapeutic effects of elongated needle and warming needle moxibustion with conventional acupuncture on slow transit constipation and their influences on colonic dynamics, and to discuss the working mechanism of warming needle moxibustion in treatment of slow transit constipation,and in order to provide more safe and effective method for the treatment of slow transit constipation.Methods: According to the Rome III Diagnostic Criteria of Functional Constipation(FC) stipulated in 2006 as well as the inclusion criteria and exclusion criteria in Guideline for Clinical Trail of Traditional Chinese New Drugs, we selected 209 patients with slow transit constipation and they were classified according to time sequence into two groups: Treament group(elongated needle and warming needle moxibustion group, group A) and control group(conventional acupuncture group, group B). The treatment group used awn needle of 0.35mm×100mm(4 28 inch)pierce bilateral Tianshu with 50mm(2 inch) ~ 100mm(4 inch),twisting up with moxa in awn needle handle for 20 minutes,and used acupuncture needle of 0.35mm×40mm(1.5 28inch) pierce bilateral Zusanli,Shangjuxu,Zhigou,Zhaohai with conventional puncture, twisting up and retaining the needle for 20 minutes; the control group used acupuncture needle of 0.35mm×40mm(1.5 28 inch) pierce bilateral Tianshu with 25mm(1.0 inch)~40mm(1.5 inch), twisting up and retaining the needle for 20 minutes,the other acupoints and theirs operation method and retention time are the same to the treatment group.The treatment was conducted with successive needling for 30 d, 1 time /d, and a 3-month period of follow-ups. The clinical symptoms and constipation scores of patients in the two groups before, during and 3 months after treatments were recorded indetail, and the effective rates of the two groups were calculated. SPSS 13.0statistical software was used to analyze the obtained data. Single factor variance analysis of grouped data, row×column chi-square test, repeated measures ANOVA and t test were applied, and P<0.05 was considered to be statistically significant.Results:1 Observation of therapeutic effects1.1 short- term results: The overall response rate(ORR) of elongated needle and warming needle moxibustion group was 92.45%, and that of conventional acupuncture group was 79.61%. Comparison between the two groups showed P<0.05.Therapeutic effects during the 1st, 2nd and 3rd month of follow-ups: The ORR of elongated needle and warming needle moxibustion group was 88.68%,and that of conventional acupuncture group was 70.87%. Comparison between the two groups showed P<0.05.2 Observation of colonic transit test: For the elongated needle and warming needle moxibustion group, the 24 h, 48 h and 72 h number of markers remaining in the intestinal tract were 16.02±3.12, 7.43±2.87 and 2.96±4.90, respectively.For the conventional acupuncture group, they were 19.44±4.23, 11.39±3.02 and 6.59±6.02. Comparison between the two groups showed P<0.05.3 Wexner constipation scores: Both elongated needle(in combination with warming needle moxibustion) and conventional acupuncture could improve the scores such as defecation frequency, defecation difficulty, defecation completeness, pain in defecation, defecation time, defecation aid and defecation failure, while elongated needle(in combination with warming needle moxibustion) had a more marked effect. After treatment, the scores of the elongated needle and warming needle moxibustion group was 6.54±3.83,whereas the scores of the conventional acupuncture group was 8.59±7.65,Comparison between the two groups showed P<0.05.Conclusion:1.Both elongated needle(in combination with warming needlemoxibustion) and conventional acupuncture could effectively reduce the patient’s constipation score,and the former method is superior to the latter,indicating that the former therapy can better improve the discomfort of patients.2.Both elongated needle(in combination with warming needle moxibustion) and conventional acupuncture are effective for slow transit constipation.However, elongated needle(in combination with warming needle moxibustion) had a more significant clinical effect with better long-term efficacy.3.Both elongated needle(in combination with warming needle moxibustion) and conventional acupuncture are effective for slow transit constipation.However, elongated needle(in combination with warming needle moxibustion) had a more significant clinical effect with better long-term efficacy.In conclusion,Awn Needle Acupuncture Moxibustion in acupuncture 、awn needle、Moxibustion in one,is a safe、effective、economical and no side effect of treatment, it is worthy of clinical application and expansion.
