OBJECTIVE: To explore the development trend of micro incision phacoemulsification cataract surgery and the effect of small incision extracapsular cataract extraction and coaxial microincision phacoemulsification cataract surgery on visual quality.METHODS: Study comprised of 104 cataract patients who underwent small incision extracapsular cataract extraction surgery(ECCE)and had lens implantation and 143 patients who underwent coaxial micro incision cataract surgery(MICS)from 2011-01-01 to 2011-12-30 at Department of Ophthalmology,First affiliated hospital of Fujian Medical University using a1.8mm clear corneal incision with implantation of foldable intra ocular lens(IOL)from Bausch& Lomb. All the surgeries were performed by 2 experienced surgeons on both procedures.Preoperative and postoperative visual acuity after 3 months was calculated.Corneal status was also observed for surgical induced astigmatism.RESULT: The patients were examined preoperatively and 3 months postoperatively. No intraoperative or postoperative complications were seen in any patients. The visual outcomes were significantly different before and after surgery(P=0.008).In statistical analysis the mean visual acuity before and after small incision ECCE was 0.14±0.11 and 0.53±0.23 respectively, similarly the mean visual acuity before and after phacoemulsification was 0.21±0.13 and0.53±0.21 respectively.Improved visual acuity of 0.3-1.0 was obtained by 88.11% patient following phacoemulsification while the percentage was 77.86 following small incision ECCE group. The astigmatism of postoperative micro-incision phacoemulsification group was significantly lower than that of small incision ECCE group. The non-parametric Mann-Whitney U test of the grade data,P <0.01,the difference was statistically significant.CONCLUSION: Cataract surgery has always been an evolving process and will remain the same in future. Both the surgical procedures showed no intraoperative or postoperative complications at all. There was no significant difference statistically in improved postoperative rate of visual acuity,however,micro-incision phacoemulsification cataract extraction and intraocular lens implantation can effectively shorten the number of hospital stay, have faster postoperative incision healing time, and less surgery induced astigmatism.
