Objective:1. through the investigation and interviews, to understand the current status of medication adherence of elderly patients with chronic diseases in Fujian province, and discussion the influencing factors.2.to discuss the effect of drug Self-Management mode to improve the medication compliance in MCI merger of chronic diseases patients with VRF.Method: 1. Random cluster sampling method was used to recruit 502 elderly patients with chronic diseases who were in Long-term Care Facilities of Fujian. To understand the current status and explore influencing factors of medication adherence of elderly patients with chronic diseases in Fujian province by medication adherence scale questionnaire investigation. 2. To recruit 10 elderly patients with chronic diseases who were in Fujian Province Hospital Outpatient department. And taking face to face respondents for in-depth interviews, to further confirm the influencing factors of medication adherence of chronic diseases. 3.To recruit 120 MCI merger of chronic diseases patients with vascular risk factors, and randomly divided into an intervention group or control group(n = 60). Control group patients was given routine nursing. Intervention group in addition to the regular hospital ward nursing, also get intervention training of drug self-management. The effect of intervention were assessed by testing changes in medication adherence.Results: 1.the current status of medication adherence of elderly patients with chronic diseases in Fujian province is not ideal, poor medication compliance of patients account for 44.4%, medication adherence to good accounted for only 5.4%. Logistic regression analysis showed that the degree of education, whether there are symptoms, course of diseases is the most important influence factors of medication adherence.2. The influence factors of chronic diseases patients’ medication compliance including drugs, patient’s factors and social factors.3. After the intervention one week, one month and three month, intervention group patients had significantly higher score at medication adherence than intervention group patients(P<0.05).There was interaction between intervention and time(P<0.05). Family burden of intervention groups compared with before significantly reduce(P<0.05). Intervention group have significantly lower blood sugar, glycosylated hemoglobin, Blood pressure and blood lipid compared with before(P < 0.05).Conclusion:1.the current status of medication adherence of elderly patients with chronic diseases in Fujian province is not ideal. The degree of education, whether there are symptoms, course of diseases is the most important influence factors of medication adherence.2.The influence factors of chronic diseases patients’ medication compliance including drugs, patient’s factors and social factors.3. Intervention can improve the medication compliance of patients with MCI merger of chronic diseases, reduce the burden of family, alleviate the patient’s condition.
