Objective: To understand the health status and long-term needs of the home-based elderly in Lingshui County,and to further explore the factors influencing the degree of long-term care needs of the home-based elderly.Methods: Stratified random sampling method was used to investigate 911home-based elderly people in Yelin Town,Yingzhou Town,Xincun Town,Wenluo Town,Qunying Township and Nanping Farm in Lingshui County.The long-term care needs assessment of the elderly at home is carried out in terms of nursing care,medical care,spiritual comfort and social support.Results: 1.A total of 911 people were investigated in this study,and the proportion of females(514 people)was higher than that of males(397 people);Age ranges from 60 to 100,with an average age of 71.99 ? 7.540.The overall characteristics of the elderly at home in Lingshui County are that most of them have self-care ability,medical and old-age security,and good living habits.Health examination,health education and medication guidance were the top three needs for long-term care for the elderly who could take care of themselves and the semi-self-care elderly.The needs of the elderly who cannot take care of themselves at home are daily life care,medical care and accompanying chat in the first place,lifelong care in the second place,psychological counseling,participation in cultural and sports activities and social interaction in the third place.2.In the assessment of long-term care needs,the entropy weight method was used to calculate the proportion of 17 three-level indicators for long-term care needs.It was found that the elderly at home had the highest demand for medical care,followed by social support and spiritual comfort,and daily life needs with the lowest weight proportion.3.The results of single-factor Kruskal-Wallis H test showed that Age,household registration,the main economic source,medical insurance,and the degree of care,chronic disease situation,the kinds of chronic disease,hypertension,diabetes,family history of cerebrovascular disease,medication compliance,and fall(3 months or less),smoking,drinking,exercise,eat breakfast times,food tastes of sour and sweet,the regularity of sleep,the sleep time and sleep quality,a total of 22 variables P<0.05,the difference was statistically significant.Pair-to-case comparison of significance levels adjusted by Bonferroni method showed that at least two groups were statistically significant among age group,type of health insurance,degree of self-care and medication status.4.Ordered Logistic regression analysis showed that age,main economic sources,medical insurance category and self-care degree all had an impact on the degree of long-term care demand(P<0.05).Conclusion: 1.On the whole,the health status of the elderly at home in Lingshui County is good,and they can basically take care of themselves.The prevalence of chronic diseases is about 50%,lower than the national average(60%).2.The highest demand for long-term care is medical care and social support,especially health education,regular health check-up and medication guidance in medical care;3,should be based on evaluation classification for different levels of the elderly in long-term care services,the household,community,institutions based on endowment patterns in long-term care service content,on this basis,for half of disability and disability groups provide exclusive service,improve the old man that occupy the home,especially in old age and disability population old age happiness.
