Research background:Drug theraPy is one of the main treatment for elderly Patients with chronic diseases. Good medication adherence gets good treatment and Prognosis of Preconditions. Domestic and international studies have shown that elderly Patients with chronic diseases exist in the wrong medicine, missing a dose, withdrawal and other non-adherence behaviors. The literature suggests that the good medication adherence in the rural areas was only 18.9%. Medication adherence has become the imPortant influential factors of theraPeutic efficacy. Present data demonstrates that self-management intervention can imProve the health behavior and life quality of high blood Pressure, diabetes, which has been gradually carried out in our urban communities. Rural aging is more serious than the city, couPled with the limited medical resources in rural areas, it is necessary to exPlore the self-management intervention for imProving medication adherence of the elderly chronic Patients in the rural areas.Objectives:To evaluate the effects of self-management invention for the elderly chronic Patients’ safe medication and medication adherence.Methods:From January 2013 to October, convenience samPling of hyPertension, diabetes, coronary heart disease Patients from a town hosPital in Li Cheng District of Jinan, who need take medication at home,82 Patients were recruited eventually. The samPles were randomly assigned to an intervention grouP (IG n=41) or control grouP (CG n=41). Patients from the IG received self-management intervention, with Self-efficacy as a guide. Through collective teaching, follow-uP, social suPPort, consulting services, medication reminders and other interventions, Patients understand more about the disease knowledge and self-management interventions. Patients are encouraged to develoP their own Plans and objectives and try to comPlete in order to obtain direct experience. Indirect exPerience obtained through model demonstration. By way of follow-uP and face-Q, verbal Persuasion can imProve the Physical and mental state of the Patient, Patients from the CG only receive usual care. After the intervention for 3 months and 6 months respectively, the safe medication knowledge questionnaire and MMAS-8 were used to evaluate the safe medication knowledge and medication adherence.Results:The 82 Patients were observed over 3 months, Recovery of 100%. (1) The scores of basic knowledge of drugs, scores of drug characteristics, scores of Prescribed medication for the intervention grouP were higher than that of the control grouP (P< 0.05). (2) Using MMAS-8 to evaluate the medication adherence, no difference was found in the medical resource utilization. The 77 Patients were observed over 6 months, Recovery of 93.9%. (1)The score of safe medication showed significantly difference between grouPs (P< 0.05). (2) Using MMAS-8 to evaluate the medication adherence, no difference was found in the medical resource utilization.Conclusions:The self-management intervention can imProve the safe medication knowledge and medication adherence, Providing an effective method for the management on chronic Patients.
