A Survey On The Satisfaction Of NCMS Of Shanxi Province

Objective:This study specially focused on the satisfaction about the New Cooperative Medical Scheme(NCMS). Through statistically analyzing the data of satisfaction about the NCMS, this paper aims to discussing the problems existing in the NCMS and putting forward the suggestions and countermeasures for them, providing the reference to the sustainable development of NCMS in Shanxi province.Methods:500 cases who hospitalized in the tertiary hospital of Shanxi province with the participation of NCMS, who hospitalized in county-level hospital with the participation of NCMS, and who were healthy peasant with the participation the NCMS were surveyed through the method of random sampling. With the self-made questionnaire, the satisfaction of those peasants participating the NCMS was collected by face-to-face interview with the peasant, who had the experience of reimbursement.All data was analyzed by using the SPSS19.0 software.Results:⑴We sent 503 questionnaires in total and 500 residents were included in this study to analyze, the recovery was 99.4%.⑵ The reliability and validity analysis of this self-made questionnaire: the Cronbach α coefficient of the financing, treatment, reimbursement, medical insurance agency, and publicity are 0.730, 0.920, 0.908, 0.867, and 0.856 respectively.For the correlation coefficient, all the items of the satisfaction of financing(CZ) are greater than 0.75; there are 17 items of the satisfaction of treatment(JY) greater than0.52; all the items of the satisfaction of reimbursement(BC) are greater than 0.59; 4items are greater than 0.80 in the satisfaction of medical insurance agency(JB); and all the items are greater than 0.80 in the satisfaction of publicity(XC). This questionnaire has a good discrimination for the composition of ICC matrix is good.⑶ The satisfaction of financing and medical insurance agency are higher compared with other dimensions. The final scores of financing, treatment,reimbursement, medical insurance agency, publicity, and general satisfaction are(10.870 ± 1.972),(65.390 ± 16.811),(36.600 ± 9.121),(10.508 ± 0.828),(9.020 ±2.767),and(3.620 ± 0.785) respectively. The univariate analysis shows that, the satisfaction of female is higher than that of male in the publicity of NCMS; the satisfaction is higher with the age increasing; the deeper the participants know the NCMS,the more they are satisfactory in the publicity of NCMS. The structural equation model shows that,the satisfaction of treatment has the most the most effect on the general satisfaction, then the satisfaction of reimbursement, the satisfaction of publicity, the satisfaction of medical insurance agency, and the satisfaction of financing.Conclusions:The peasants participating the NCMS in Shanxi province are satisfied generally.However they are not satisfied with the high cost of designated medical institution;they are not satisfied with the medical service of village clinic, thinking the function and role of it was not played fully; they hold that the scope of reimbursement and the hospitalization reimbursement in outpatient clinic are still low; they think the publicity of NCMS among the designated medical institution is not sufficient which attributes to the peasants have less knowledge about the NCMS. The regulation and management of designated medical institution of NCMS should be strengthened and the publicity of NCMS should be increased, improving the satisfaction of peasants participating the NCMS.
