Objective:This study is to aim at learning nursing work environment satisfaction level in the 3A-level general hospital in Shanxi Province and exploring its impact factors.These nurses are in different departments and have different ages, different care ages, different titles, different duties, different education, different personnel conditions etc. The flaws and deficiencies are as a starting point for continuous nursing quality improvement and provide a theoretical basis for nursing managers' further effective intervention measures to improve nursing work environment satisfaction, stabilize the nursing team and improve nursing quality. While this study is to aim at conducting experimental study on a large sample in order to further verify reasonability and feasibility of nursing work environment evaluation index system that Bi hongmei built preliminarily.Methods:The nursing work environment satisfaction questionnaire was formed through an extensive literature research, nursing work environment evaluation index system that Bi hongmei built reference and nursing experts discussion. The questionnaire is divided into seven dimensions:nurse participation in hospital affairs, work safety, work repay, management patterns, shifts in principle, professional and personal development, teamwork and has a total of 44 entries. Recovery rate and pass rate are used to evaluate feasibility of the questionnaire. The reliability of the questionnaire is considered in the internal consistency angle and analyzed using Cronbach's a coefficient. Exploratory factor analysis and confirmatory factor analysis are used for construction validity analysis.These representative and cooperational six 3A-level general hospitals are choosed in urban districts with 3A-level general hospitals in Shanxi Province. This study investigates 679 clinical nurses' work environment satisfaction in the six 3A-level general hospitals using convenience sampling and proportional sampling method. All information data obtained through investigation are input SPSS13.0 statistical software to analyze. The frequency and composition than describe the nurses' the general information; The median and interquartile range describe the concentration and dispersion tendency of nursing work environment satisfaction scores; The Kruskal-Wallis Test analyzes and explores whether different nurses' work environment satisfaction scores have significant differences; An average rank show different nurses' work environments satisfaction extent.Results:1. Questionnaire evaluation(1) Feasibility:In this survey, a total of 700 questionnaires were issued and 684 questionnaires were recovered, so the recovery rate is 97.71%. Excluding five unqualified questionnaires,679 questionnaires were qualified and the pass rate is 99.27%.(2) Reliability:After testing, the overall Cronbach's a coefficient of questionnaire was 0.964, the Cronbach's a coefficient of the dimensions rang is between 0.779 and 0.901, which are at 0.7 above. The questionnaire has a higher reliability.(3) Validity:The principal component analysis method is used for exploratory factor analysis. The questionnaire is divided into seven principal components that can reflect the 63.30% information amount. The confirmatory factor analysis is used to verify the structure of the questionnaire. The model X2= 4068.93, df=881,X2/df= 4.62 and the value of X2/df that is less than five shows a better model fit. Estimated mean square error approximation RMSEA= 0.073 and the value of RMSEA that is less than 0.08 shows a good model fit. Factor analysis results are basically consistent with the questionnaire structure. The questionnaire has a better construction validity.2. The impact of population sociology factors on satisfaction scoresThis study shows that the overall satisfaction scores of nurses who have different ages, care ages, marital status, health status, dapartments have differences statistically. While the overall satisfaction scores of nurses who have different job titles, duties, personnel status have no differences statistically. However, the satisfaction scores differences exist in specific dimension. The overall satisfaction scores of nurses who have different final education has no difference statistically, there were also no differences in specific dimension scores.Overall 20-29 age group (age care=1-5 years) nurses'satisfaction level is higher than 30 years (age care> 5 years) nurses'satisfaction level; Single nurses'satisfaction level is higher than married nurses'satisfaction level; Good health nurses'satisfaction level is higher than poor,general health nurses'satisfaction level; Junior title nurses'satisfaction level is higher than intermediate title nurses' satisfaction level, senior nurse'satisfaction level is higher than junior nurses'satisfaction level; Head nurses'satisfaction level is higher than nurses' satisfaction level; Official compiled nurses'satisfaction level is higher than contract nurses' satisfaction level; All departments satisfaction decreased in the order:outpatient,operating room,internal medicine,obstetrics and gynecology,pediatrics,emergency and critical,surgery.Conclusion:1. Upon investigation, the overall nursing work environment satisfaction is lower in 3A-level general hospital in Shanxi Province. Nurses'satisfaction level in the seven dimensions were ordered in the following:shifts in principle; management patterns,professional personal developmentï¼›participation in hospital affairs,work safety,teamwork,work repay.2. A large sample of experimental study on nursing work environment evaluation index system that Bi hongmei built preliminarily was conducted and further verified reasonability and feasibility of evaluation index system in a large area. After testing, Cronbach's a coefficient of the questionnaire having higher reliability was 0.964. Exploratory factor analysis and confirmatory factor analysis results are basically consistent with the questionnaire structure. The questionnaire has a better construction validity. On the whole, nursing work environment evaluation index system has better reliability and validity.
