A Study On The Transformation Strategy Of The Public Secondary-level General Hospital In Cities

At present, some unreasonable phenomenon still occur in city medical and health resources allocation, the country had introduced a number of new medical reform policy, but failed to ease the masses whose problem is "high expense and difficulties in medicine ". Along with social progress, economic development, people’s living standards improve, and the reform of medical insurance policy, people are free to choose the hospital, especially in large and medium-sized city with abundant medical resources,such cities as Beijing, Shanghai, Guangzhou and so on whose traffic are convenient, as well as the enhanced medical need of people, making the public third-level hospitals in cities overcrowding and the source of the disease in the secondary-level hospitals gradually shunted. However, another extreme phenomenon namely there are different degrees of idle about medical and health resources of city emerged. The phenomenon of the public secondary-level general hospital is especially serious, the supply and demand do not match, the contradiction is very outstanding, the reason is the third-level medical and health service system has a structural problem. In order to rationally allocate health resources of city, alleviate the problem of ‘high expense and difficulties in medicine’, the new medical reform policies encourage the city to convert third-level medical and health service system into secondary-level bidirectional medical and health service system so that to adapt to the rapid development of medical market demand, but also faces with the trouble of the public secondary-level general hospital in cities repositioning and transformation of the development. How to overcome difficulties of secondary-level general hospitals in cities, find their own characteristics, speed up the transition, achieve sustainable development, which has become the focus of widespread social concern.In this paper, I consult massive related reference about the secondary-level hospital and the public secondary-level general hospital in cities in the survival of the status quo, development and transformation, consider the new policy of medical reform introduced by the Ministry of health and the big city, analyze health information statistical yearbook in recent years, using literature research method, case analysis method, SWOT analysis method and ratio analysis method, I conclude that the public secondary-level general hospital in cities are in the survival plight, the transformation direction is unknown, development is arrested, which are showed in the following four aspects: 1. the number of secondary-level hospitals indicates negative growth phenomenon; 2. the number of beds in growth rate of secondary-level hospitals is much lower than that in the other level hospitals; 3. the growth rate of served people in secondary-level is far lower than that of other level of hospitals; 4. the using rate of beds is low and the phenomenon of empty bed is obvious. There are five aspects of reasons why the secondary-level hospitals are in a dilemma, the difficulty of self-orientation, the unclear direction of transformation, personnel shortages, funding shortage, locational disadvantage, and adverse policy guide.How to get out of the plight? This paper has put forward several strategies. The leaders or the policymakers of the public secondary-level general hospital in cities should use the SWOT analysis method to analyze the internal and external advantages as well as the factors that hinder its development, they can draw up the transitional development strategies of the hospital by using the SWOT analysis strategy combinations and Porter’s theory of competitive strategy, The hospital can successful transform by finding their own market position, strengthening self-construction and enhancing the core competitiveness. This article has detailed three typical development strategies. The public secondary-level general hospital in cities can choose a suitable development strategy according to their own situation as well as the policy guidance. Hospitals with strong comprehensive strength can promote to the third-level hospital via policy support channel and become a regional medical center; Hospitals with weak comprehensive strength can transform into the community health service centers which provide the basic health service; Hospitals with specialist brand advantages can transform as the features specialist hospital, implemented " large-special and small-general ".This study intends to seek the strategy of the transformation and development of the secondary-level general hospital in cities. By analyzing the survival status of the secondary-level hospital in cities under the background of new medical reform, thinking about the transformation problem of the secondary-level hospitals especially of the public secondary-level general hospital in cities, analyzing the theoretical basis of transformation, and ensuring the specific measures of the implementation of the strategy, so these can accumulate the experience for the transformation of the public secondary-level general hospital in cities, provide support for the reform mode which have the expansion values in the secondary-level general hospital in cities and provide reference for the other secondary-level general hospitals needed to transform, and also provide the theory and certain work basis for the administration management department to layout of city medical and health resources, and to plan medical service system of national health.
