Effects Of Ganzhikang Granule On Nonalcoholic Steatohepatitis Lipopolysaccharide,leptin And Insulin Resistance Index

Objective : This research from TCM syndrome, liver function, triglyceride, total cholesterol,lipopolysaccharide, leptin, insulin resistance index, color doppler grading, liverstiffness, body mass index, total efficiency etc to evaluate ganzhikang granule treating nonalcoholic steatohepatitis(stagnation of phlegm and blood stasis) clinical curative effect and security of the scheme.Methods : Selected 70 patients in clinic order number, using completely randomized digital table method, according to the 1:1 ratio, the patients were divided into treatment group and control group. The treatment group with 35 cases, The control group with 35 cases. The two groups were treated with foundation treatment which contain low calorie, low fat and vitamin rich diet,avoid wine, greasy etc; proper aerobic exercise, to sweat for the degree, positive weight control. Based on the foundation treatment, the control group was given Silibinin Capsules oral; Based on the control group, the treatment group was given ganzhikang granule. Two group patients were treated for 12 weeks for 1 courses. Observation patients before and after treatment of TCM syndrome curative effect, function, lipopolysaccharide, triglyceride, total cholesterol, leptin, insulin resistance index, color doppler grading, liver fat attenuation, body mass index, total efficiency, safety indicators and adverse events.Results :(1) The comprehensive curative effect :according to the comprehensive curative effect evaluation criteria, the treatment group 13 cases were markedly effective, effective 18 cases, invalid 4 cases, the total effective rate was 88.6%; the control group 7 cases were markedly effective, effective 15 cases, invalid 13 cases, the total efficiency of 62.9%. Rank sum test analysis, Z=-2.435, P=0.015<0.05, compared to the two groups have statistical difference.(2)The TCM syndrome: according to the standard of curative effect evaluation of TCM syndrome to evaluate and analyse, in the treatment group 2 cases recovered, 12 cases were markedly effective, 19 cases effective, invalid in 2 cases, the total effective rate was 97.06%; in the control group 0 cases were cured, 8 cases markedly effective, effective 16 cases, 11 cases ineffective, the total efficiency is 68.6%; The Rank sum test analysis,Z=-2.574, P=0.010<0.05, compared to the two groups have statistical difference.(3) Effect of liver function and other related indicators: at the end of treatment,the level of liver function(ALT,AST), blood lipid(triglyceride, total cholesterol),lipopolysaccharide, leptin, insulin resistance index, liver fat attenuation, body mass index of two groups were significantly decreased, the treatment group decreased obviously, there was statistically significant difference between the two groups(P<0.05).(4) The imaging effect: according to the standard of curative effect evaluation of color doppler to evaluate and analyse, at the end of the treatment, in the treatment group 8 cases recovered, 14 cases were markedly effective, 11 cases effective, invalid in 2 cases, the total effective rate was 94.3%; in the control group 3 cases were cured, 8 cases markedly effective, effective 15 cases, 9 cases ineffective, the total efficiency is 74.3%; The Rank sum test analysis, Z=-2.930, P=0.003<0.05, compared to the two groups have statistical difference.(5) Security: safety indexes of 70 patients before and after treatment,such as blood, urine, stool routine, occult blood test and ECG, kidney function(urea,crea) showed no obvious abnormality. 35 cases in the treatment group, 3 cases of constipation, diarrhea in 2 cases, 35 cases of control group patients had no adverse reaction.Conclusion : Ganzhikang Granule in the treatment of non alcoholic steatohepatitis(phlegm and blood stasis in the interior) curative effect, can significantly improve the syndromes of TCM in patients with non alcoholic steatohepatitis, and it can effectively improve the patient’s liver function, triglyceride, total cholesterol, lipopolysaccharide, leptin, insulin resistance index, color doppler grading,liver fat attenuation, body mass index, the total effective rate is high, and the clinical application is safe and effective, effective prescription combination for treating nonalcoholic steatohepatitis, its mechanism may be related to the decrease of lipopolysaccharide, leptin, insulin resistance index.
