Objective : To analyse the syndrome differentiation and and distribution of traditional Chinese medicine(TCM) of Ulcerative Colitis(UC) based on “Deficiency, Toxicity and Blood Stasis” theory,in order to develop treatment plan on three stages(attack, recovery, and stabilization stage), and to evaluate the clinical efficacy of Chinese medicine and provide the basis for clinical treatment and research of UC.Methods:According to the UC western diagnostic criteria,76 cases in active in mild, moderate and the remission of UC, were randomly divided into two groups, 38 cases of treatment group and 38 cases of control group. Treatment group were treated based on the theory of “Deficiency, Toxicity and Blood Stasis”in TCM :Attack stage patients were given Jiedu Shenghua Decoction for oral administration and Shengji Yuyang 1st Decoction for retention enema; Recovery stage patients were given Anchang Yuyang Decoction for oral administration and Shengji Yuyang 2st Decoction for retention enema;Stabilization stage patients were given Guyuan Huoxue Decoction for oral administration. While control group were given the salad oxazine sustained-release particles(etiasa) 2-4 g/d.After 8 weeks of treatment, observe the clinical efficacy, TCM symptoms, effects and pathological lesions of colonic mucosa and serologic test(ESR、CRP、D-dimer) efficacy evaluation, follow-up for half a year to observe the relapse.Results:① short-term effectThe clinical comprehensive efficacy, the total effectiveness of treatment group was 92.11%, while control group 86.49%,the difference was not statistically significant(P >0.05).The total effectiveness on TCM syndrome of treatment group was 89.47%, while control group 78.38%,the difference was statistically significant(P < 0.05).Colonic mucosa lesions efficacy compared between the two groups was statistically significant(P <0.05); colon comparison of the efficacy of mucosal pathology and ESR、CRP, the difference was not statistically significant(P >0.05).② long-term effect:Follow-up six months later,2 cases of treatment group recurred, the recurrence rate was 5.71%, while 10 cases recurred in control group, the recurrence rate was 31.25%,the difference was statistically significant(P <0.05);the treatment group recurrence were all mild phase, while the control group recurrence got 4 cases with mild phase,6 cases with moderate phase, the difference was statistically significant(P <0.05).Conclusion: After treatment and long-term follow-up,the effects of syndrome differentiation of TCM based on the “Deficiency,Toxicity and Blood” theory were better than the control group, and can reduce recurrence. Its mechanism may be related to immune regulation, anti-inflammatory antibacterial, improving blood hypercoagulable state, and repairing the intestinal mucosal barrier.
