The Clinical Observation Of Xiangsha Liujunzi Decoction And Danggui Buxue Decoction Treat With Acute Leukemia Adverse Reaction After Chemotherapy

Objective: To observat the clinical effects of Xiangsha Liujunzi Decoction and Danggui Buxue Decoction treat with acute leukemia adverse reaction after chemotherapy. And discuss the action mechanism from two aspects of theory and clinical.Method: 1.60 acute leukemia patient with chemotherapy were treated in hematology department of Wuhan No.1 Hospital in March 2013 to December 2014.TO see a doctor order for reference,in accordance with the random number table,were divided into observation group and control group.Observation group and control group is 30 cases.There was no evident difference compared with two sets of datas in general information,the gender,age,condition,duration, etc(P>0.05). 2.Two groups adopt based chemotherapy regimens:The treatment group and the control group used the same basically plan.Acute myeloid leukemia(AML)using the HA solution.(HHT 2-4mg/d,iv,1-7 days;Arac 100-150mg/(m2.d),iv,1-7 days;2 weeks rest betweenchemotherapy sessions);Acute lymphoblastic leukemia(ALL) using the VDCP solution(VCR 2mg/time,iv,1,8,15,22 days;CTX 60-80mg/time,iv,1,15 days;DNR 40mg/m2,iv,2-4 days;Prednisone 40-60mg/d,1-14 days,redution from the 15 th to 28 th,2 weeks rest between chemotherapy sessions).Support therapy including control, treatment of bleeding and nutritional support,etc.Chemotherapy periods is 28 days/times,persistent observation is 2 chemotherapy periods. treatment group give based chemotherapy regimens+metoclopramide+ Xiangsha Liujunzi Decoction and Danggui Buxue Decoction:Taizishen 15 g,Baishu 10 g,Fuling 10 g,Gancao 10 g,Chenpi 6g,Banxia 10 g,Sharen 6g,Muxiang 10 g,Huangqi 60 g,Danggui 10 g.In the add and subtract:Deficiency who added Nvzhenzi 20 g,Hanliancao 20g;Yang deficiency added Fuzi 6g,Ganjiang 8g,Poor appetite added Shanzha 15 g,Maiya 10 g,Shenqu 15 g,Insomniac added Hehuanpi 10 g, Yejiaoteng 15 g,Bleeder added Xianhecao 30 g.(Above all traditional Chinese medicine drug granules,batch number 1310241-1411247).TCM began to use in 3 days before chemotherapy,1 agent/day,2 times/day,for 4 weeks.Metoclopramide(Approved by H12020967) 20 mg,muscie injection before chemotherapy 20 min.Control group give based chemotherapy regimens+metoclopramide:Metoclopramide(Approved by H12020967) 20 mg,muscie injection before chemotherapy 20 min.Chemotherapy periods is 28 days/times,persistent observation is 2 chemotherapy periods.From clinical observation for two groups of patients after chemotherapy drug treatment gastrointestinal reaction degree effectively,KPS score change,Hb and WBC and PLT and the clinical comprehensive efficacy before and after treatment,assess clinicalefficacy of Xiangsha Liujunzi Decoction and Danggui Buxue Decoction treat with acute leukemia adverse reaction after chemotherapy.The three great regular tests,liver and renal functions and ECG(before and after treatment)was continuously recorded for two groups of patients in the process. 3.Statistical analysis method:Data were analyzed by SPSS19.0 software.Count data using x2 test,measurement data using t test.Result: 1.Before treatment two group gender,age distribution, classification of acute leukemia and KPS score comparison,results suggest two groups of patients with general data difference is not significant(P>0.05),with comparable. 2.Treatment after chemotherapy of gastrointestinal reaction efficiently:After treatment,two groups have improved the gastrointestinal symptoms,including nausea and vomiting.TCM treatment group better than control group in improving the degree of nausea and vomiting score,differences are statistically significant(P<0.05),TCM syndrome differentiation and treatment scheme has significant advantages in reducing the gastrointestinal adverse reaction. 3.KPS score comparison:KPS score is stable in TCM treatment group,and decline in control group.Compare with KPS score pretherapy and post-treatment,TCM treatment can stable behavior of patients. 4.Myelosuppression comparison:Through the T test analysis, There is no difference on the three great regular tests prior treatment(P>0.05).There is difference on Hb and PLT of TMC Group before and after treatment(P<0.05),and there is nodifference on WBC(P>0.05).It is accounts for TCM group have positive regulatoy on myelosuppression after chemotherapy. There is difference on the three great regular tests prior and after treatment in control group(P<0.05).It is accounts for based chemotherapy regimens have marrow suppress.Compare with he three great regular tests post-treatment in two groups,with statistical significance(P<0.05).TCM treatment group better than control group in improving the amount of Hb,WBC,PLT. 5.Comprehensive comparative effedtiveness:TCM treatment group of 30 cases,19 cases were complete response,9 cases were effective,2 cases were invalidation,the total rate was 90%;control group of 30 cases,14 cases were complete response,6 cases were effective,10 cases were invalidation,the total rate was 66.3%;Through the chi-aquare test analysis processing,it is indicate the data between the two groups statistically significant difference(P<0.05).It follows that the TCM treatment group is better than westerm medicine control group.Conclusion: The study suggest Xiangsha Liujunzi Decoction and Danggui Buxue Decoction is safe and effective for the treatment of adverse reaction on chemotherapy.The formulas can improve the efficient of adverse reaction treatment,stable KPS score,improve bone marrow suppression and better than the control group on comprehensive curative effect,and deserve a further research and expand.
