ObjectiveThe study investigates the clinical nurses, in order to assess the status of job burnout, self efficacy and sleep quality, and to find out the relationships between job burnout and self efficacy or sleep quality, and then explore the factors of job burnout. Through the investigation on the nurses, the research can provide the theoretical basis to prevent or reduce the job burnout. MethodsAll of the 260 research subjects were from surgical, medical, obstetrical and pediatrics departments in 5 hospitals, which are all Tertiary comprehensive hospitals in Kaifeng, Zhengzhou, Zhoukou and Nanyang. By using the method of cross-sectional study, the nurses were chosen to fill out the questionnaire survey from November 2013 to May 2014. The study tools included: ①The general information questionnaire to assess population statistics; ② The Maslach Burnout Inventory Human Services Survey(MBI-HSS) to assess the current situation of job burnout; ③ The general self efficacy assessment to evaluate the status quo; ④ Pittsburgh Sleep Quality Index scale to assess sleep quality of nurses.Using the softwire of EpiData 3.1 establish a database and the software of SPSS 17.0 for analysis, in which the level of the test α was 0.05, thus P values of it were two-tail probability. Assessed the status of job burnout, self efficacy and sleep quality by descriptive statistics, t test and analysis of variance, and explore the relationships between the burnout and self efficacy or sleep quality by correlation analysis, and the influence factors of job burnout by multiple linear regression analysis. Results1. The current status of job burnout, self efficacy and sleep quality of clinical nursesThe mean scores of emotional exhanstion dimension, depersonalization dimension and personal accomplishment dimension were respectively(27.17 ± 6.78),(11.03 ± 4.12),(25.06 ± 6.13). 84.7% of nurses appear burnout phenomenon,in which the proportion of mild, moderate, severe job burnout were respectively 28.1%, 27%, 29.6%. Comparing job burnout scores with the job burnout norm of Hangzhou, the emotional exhaustion scores and the depersonalization scores were both higher than the norm, and the personal accomplishment scores were lower than the norm(P<0.05), the difference was statistically significant.The mean score of self efficacy of clinical nurses was(2.44±0.33), in which the proportion of mild, moderate, severe job burnout were respectively 9.6%, 86.5%, 3.9%. The self efficacy scores was lower than the self efficacy norm when compared with the norm(P<0.05), the difference was statistically significant.The mean score of sleep quality of clinical nurses was(8.33±2.41), the percentage of sleep quality which is low and good were respectively44.6%, 55.4%. The sleep quality scores were higher than the sleep quality norm when compared with the norm(P<0.05), the difference was statistically significant.2. The scores comparision of job burnout and self efficacy with different characterristics of chinical nursesThe nurses of working in the city of Kaifeng scored higher than the other cities score in emotional exhaustion dimention(P<0.05), the married nurses scored higher than unmarried nurses in depersonalization dimention(P<0.05), the nurses of 6~9 year seniority scored higher than the others in depersonalization dimention(P<0.05); the nurses of 10~27 year seniority scored higher than the others in accomplishment dimention(P<0.05),the nurses of surgical department scored higher than the others in accomplishment dimention(P<0.05).The nurses of 30~47 year-old group scored higher in self efficacy(P <0.05), the married nurses scored higher than the others in self efficacy(P <0.05), the nurses who had children scored higher than the others in self efficacy(P <0.05), the nurses of personnel relation which was personnel agency scored higher than the others in self efficacy(P <0.05), the nurses whose shift type were normal day shift scored higher than the others in self efficacy(P <0.05).3. The relationships between job burnout and self efficacy or sleep quality of clinical nursesThe emotional exhaustion had negative correlation with the self efficacy(r=-0.254, P<0.01), the emotional exhaustion had positive correlation with the sleep quality(r=0.310, P<0.01), the depersonalization had positive correlation with the sleep quality(r=0.312, P<0.01).4. The influence factors of job burnout of clinical nursesThe analysis showed that age, professional life, job preparation, work city, sleep quality and self efficacy are the influence factors of burnout. The proportion which sleep quality, work city and self efficacy can predict the variation of emotional exhaustion is 20.3%(R2= 0.203), the proportion which sleep quality, job preparation, professional life and age can predict the variation of depersonalization is 24.8%(R2=0.248), the proportion which city can predict the variation of accomplishment is 5.9%(R2=0.059). Conclusions1. Job burnout of clinical nurses is in medium-high level, self efficacy is in moderate level and sleep quality is in medium level, it should cause the attention of nursing managers.2. The higher the self efficacy, the lower the job burnout of clinical nurses;the better the sleep quality, the lower the job burnout.3. Age, practce period, post staffing, work city, self efficacy and sleep quality are the influence factors of job burnout in clinical nurses. Nurse managers should take measures to prevent job burnout in the light of individuality, also by improving the self efficacy and the sleep quality to reduce the job burnout.
