Present Situation Investigation Of Medical Staff Health State Of A 3A Hospital In Yunnan Province

Objective To learn medical staff health state of a 3A hospital in Yunnan Province and analyze possible factors affecting medical staff health state with the intention to provide clinical reference for improving heath state of medical staff.Methods According to the principle of stratified cluster sampling, took 350 medical personnel (including physicians, medical technicians and nursing staff) from a total of 1323 who accepted physical examination of a 3A hospital in Yunnan Province in 2014. Collected their physical examination data, performed questionnaire survey on psychological health, working pressure and working strength, conducted qualitative interview for certain medical staff and administrative staff, adopted epidata3.1 for data entry, and applied SPSS 17.0 statistical software for statistic analysis.Results1. Medical staff health state:Among the 350 medical staff, only 71 of them are totally healthy, accounting for 28.40%,279 with one or above items abnormity, accounting for 71.60%; the sorting of top ten high incidence of abnormal indicators in terms of detection rate:overweight (28.57%) is ranked the top, followed by fatty liver (18.57),hyperlipemia(16.00%), thyroid positive tubercle (15.71%), hypertension (14.00%), kidney stone (8.57%), sinus arrhythmia (8.00%), colloid cyst of the thyroid gland (7.71%), hyperglycemia (7.14%) and hepatic cyst (6.86%). Comparisons of high incidence of abnormal indicators detection rate of physicians, medical technicians and nursing staff:Through x2 test, among the top ten abnormal indicators in terms of detection rate, the detection rate differences of sinus arrhythmia and colloid cyst of the thyroid gland are not statistically significant (P>0.05), the detection rate differences of other high incidence abnormal indicators are of statistical significance (P<0.05). The detection rate of physicians is the highest, that of medical staff is the lowest; the top three abnormal indicators for female medical staff are hyperplasia of mammary glands (62.45%), overweight (25.69%) and fatty liver (17.39%); and that for male medical staff are overweight (36.08%), fatty liver (21.65%) and hypertension (20.62%). The abnormal indicator with the highest detection rate for different age groups is overweight.2. Psychological health state of medical staff:Compared with national norm, the separate score and total score of 350 medical staff SCL-90 summarization, obsessive-compulsive symptom, interpersonal sensitivity, depression, anxiety, terror, paranoia are higher. And the differences are of statistical significance (P<0.05). The overall mental health status of medical staff is lower than general population; separate score of medical staff with different genders, titles and employment forms and total score are not statistically significant (P>0.05); the SCL-90 test results of medical staff with different education backgrounds indicate that the psychological health statue of medical staff with an education background of master’s degree or above is worse than that of bachelor degree. The SCL-90 test results of medical staff with different employment experience indicate that the psychological health statue of medical staff with an employment experience over 20 years is the worst. The SCL-90 test results of medical staff with different occupations indicate that the psychological health statue of medical staff of medical technician is the worst. For those with different marital status, the psychological health statue of married medical staff is worse.3. Working pressure and working strength of medical staff:Among the 350 medical staff,68.86% medical staff felt great working pressure at present. The average daily working-hour of 51.14% medical staff is 9-12 hours. The daily average sleep time of 62.57% medical staff is 9-12 hours.62.29% medical staff need to work overtime frequently and in long term.Conclusion1. Among the 350 medical staff, The totally healthy rate is only 28.40%, and one or above items abnormity accounts for 71.60%, The medical staff health state is not optimistic.2. Compared to normal population, the medical staff psychological health state is worse and requiring concerning. Due to wicked professional environments, the psychological symptoms such as summarization, obsessive-compulsive symptom, interpersonal sensitivity, depression, anxiety, terror, paranoia and so one are more obvious than that of normal group.3. The daily working-hour for more than half of medical staff exceeds eight hours, They feel great working pressure and insufficient sleep In the investigation of medical staffSuggestion1. Reasonable distribution of medical resources, reduce the work intensity of the medical staff of 3 A hospital.2.
