Objective: On the basis of analyzing the report of annual physical examination of the medical staff in a tertiary referral hospital,learn about the health status of the target population and the trend of common diseases,investigate the risk factor of poor health,and put forward the effective suggestion for health promotion.Moreover,this study could provide the foundation for the relative departments to take specific measurements for health promotion and health care work on target population.According to these,medical staff may improve life quality and do better for human health.Methods:1 Research population: This statistical analysis of the results of 4903 person-time health examination in Hebei General Hospital from 2012 to 2016 selected the medical staff with complete information of main health examination items as the research object,excluding retirees and re-employees.2 Data collection: The basic information of hospital staff was collected from hospital personnel department,and the health examination reports of them were collected from physical examination center.The study items include height,weight,waist circumference,blood pressure,electrocardiogram,chest X-ray(less than 40 years old),chest CT(more than 40 years old),ultrasound(carotid abdomen,thyroid,bilateral carotid artery),blood routine,blood lipid,liver function,renal function,and women also include gynecological examination,gynecological pathology(cervical cytology)and breast examination.3 Research content: The disease spectrum of the target population was summed up and compared with the national norm.Longitudinal study was used to explore the dynamic changes of physical and mental health for medical staff in the past 5 years.Moreover,estimating disease developing trend,analyzing the relationships between disease and gender,age,marital status,working years,educational level,occupation position,grade,smoking history,drinking history respectively in target people,and summarizing the risk factors which could affect health status of medical staff.4 Statistical method: Stata12.0 was used for data cleaning and analysis.The minimum sample size was calculated using the exact clopper-pearson confidence interval method.The basic demographic characteristics of research population were described by descriptive statistical methods.The based growth speed and sequential growth speed of various diseases or abnormal indications were described.Univariate and multivariate stepwise logistic regression analysis was used to explore the influencing factors of health status for medical staff,and P value less than 0.05 is statistically significant.Results:1 This study shows that the five-year average missing rate of regular examination for the hospital medical staff was 18.84%,the average total abnormal index / disease detection rate was 80.11%.The situation of suffering from various diseases and abnormal index was shown below: 27.07% study subjects suffering from one kind of diseases/abnormal index,22.54% of subjects suffering from two kinds of diseases / abnormal index,13.75% suffering from three diseases / abnormal index,and 16.77% having four or more diseases / abnormal index;2 The five-year average positive rate was listed in descending order: gynecologic diseases(27.36%),digestive system diseases(27.25%),prostate disease(24.42%),abnormal liver function(23.70%),hypertension(23.19%),lipid metabolism disorder(21.37%),fatty liver(17.87%),diabetes(13.32%),abnormal renal function(9.20%),respiratory system diseases(7.85%),cardiovascular disease(7.79%),cardiovascular disease(7.79%),tumor(7.75%),abnormal electrocardiogram(6.03%),ocular diseases(4.98%);3 The dynamic changes of detection of disease / abnormal index in five consecutive years was up and down according to the different positive items,but the overall trend was upward.Except for the significantly increasing base development speed in detection rates of Diabetes,lipid metabolism and renal dysfunction,other base development speed of positive detection rates remained unchanged compared with those in 2012,showing a stable increasing trend;4 Sex,age,BMI,marriage status,education,working age,position,professional class,title,smoking and alcohol history have been compared in this study.The results were as follows: the rates of nearly all the study indication for male are higher than female,except tumor.The rates of most diseases are increasing with age,BMI and working age.The rates of some diseases are higher for clinician and administrative staff than other healthcare workers.Workers with higher grades,lower education levels had higher risk of developing diseases.There was no statistical relationship between marital status and the rate of diseases.Smoking and alcohol population had higher rates than non-smoking and non-alcohol population;5 In univariate logistic regression analysis,there were statistical association between all risk factors and the diseases except title.However,in multivariate stepwise logistic regression analysis,only age,BMI,working years,position and alcohol history were viewed as risk factors.Conclusion: This study showed the healthcare workers' health status is not very well.The prevalence rates of digestive system diseases,abnormal liver function,hypertension,lipid metabolism disorder,and gynecologic or prostate disease are relatively high.The rates of over-weight,lipid metabolism disorder,and diabetes are higher than national norm,all kinds of rates of disease were increasing totally,the positive indications were associated with sex,age,BMI,working years,position and lifestyle,which would attract great attention of relative departments,and impel them take effective interventions.
