Objective: To discuss the clinical treatment of femoral neck fractures in two different surgical methods of treatment;To the different timing of surgery in the surgical fixation impact on the clinical effect of femoral neck fracture after discussing injuries.Methods: Study in Changzhi Medical College Hospital Affiliated and economic orthopedic selected cases, 121 cases of displaced femoral neck fracture(in June 2011 to October 2012 receive treatment) retrospective analysis of cases within a given fixed total of 86 cases given total hip arthroplasty cases there were 35 cases, according to the circumstances of different surgical methods for surgery(operative time, blood loss),complications(wound infections, lung infections, ipsilateral deep vein thrombosis) and Harris score( pain, function, range of motion) comparisons. In the 86 cases of internal fixation of cases, according to the operation distance injury time(in 24 hours as the standard) and then grouping, surgical injury time away from patients ≥24 h of non-emergency surgery group(45 cases), injury to the surgical time ≤24 h As emergency surgery patient group(41 cases). Fixation patients underwent closed reduction and internal fixation. 180 days after the occurrence of complications, joint function(Harris score), fracture healing were observed. In the 86 cases of non-emergency surgery patients fixation surgery group(injury to surgery time ≥24 h)and emergency surgery group(injury to surgery time ≤24 h) were fracture healing,Harris score, more complications.Results:1 A comparison of different surgical procedures: hip group of blood loss andoperative time was significantly higher than the internal fixation group(P <0.01).2 Comparison of different surgical methods: internal fixation group compared with total hip joint function group difference(P <0.05).3 Compare three different surgical methods: internal fixation group complication rate compared with total hip group(P <0.01).4 the same surgical approach(fixed) Comparison of the different timing of surgery:(1) the rate of fracture healing: emergency surgery group than in the non-emergency surgery group high(P <0.05).(2) For the fixation of patients, emergency surgery group hip function was significantly higher than non-emergency surgery group(P <0.05).(3) the incidence of complications: emergency surgery group than in the non-surgical group had emergency low(P <0.05)Conclusions: Total hip arthroplasty for the treatment of femoral neck fractures relative to internal fixation of femoral neck fracture complications occur less joint function to be excellent, but perioperative surgical risk; select closed reduction and internal fixation surgery as much as possible within 24 h treatment can effectively reduce the incidence of complications and improve the overall effectiveness of the treatment.
