BackgroundThe health administrative departments and the medical industry are trying to find out a way of controlling health care costs as well as protecting the quality of medical care delivery model to expect to provide the general public with high quality, efficient, low-cost health care experience. As a new standardized clinical practice management, the clinical pathway has been recognized by the administrative department of health and promoted by medical institutions around the country. As one of the executive bodies of clinical pathway, the cognition and practice of the clinician for clinical pathway will directly impact on the implementation effect of clinical pathways.ObjectiveIn order to effectively guide the management capacity of the clinician, and promote an advice standardized, reasonable and sustainable development on clinical pathway, this research focused on investigating clinicians’ knowledge, belief/attitude, and practice of clinical pathway among four tertiary hospitals in Kaifeng, understanding clinicians’ knowledge and ability of clinical pathway, and exploring the impact factors of clinical pathways KAP.Methods“Clinical Pathway KAP Survey for Clinician”, designed by the researcher, was distributed to 263 clinician among four tertiary hospitals in Kaifeng who with a method of inclusion and exclusion criteria. SPSS17.0 statistical software was used for data analysis with frequency, percentage, mean, standard deviation and other descriptive statistical analysis and statistical methods with Kruskal-Wallis rank sum test, T test and multiple liner regression.Results1.The mean score of clinical pathway KAP of clinicians among four tertiary hospitals in Kaifeng were 41-145 points(115 ± 17.36), clinical pathways knowledge score were 3-18 points(14.74 ± 2.31), clinical pathways attitudes score were 20- 90 points(63.91 ± 1.11), and clinical pathways practice score were 20-38 points(36.46 ± 7.87).2.Clinicians’ clinical pathways knowledge, attitudes and practice are positively related(P<0.001).3.There are differences between knowledge, attitude scores of clinical pathway and scores of KAP for clinicians in different sections, and job titles, work experience and whether implementing the electronic system of clinical pathway(P<0.05).There are differences between knowledge, attitude scores of clinical pathway for clinicians in different positions(P<0.05).4.Throughout the multiple linear regression, the factors which affect the score of clinical pathway KAP were whether the department included internal medicine, working years of clinicians were about 10-15 years or more than 20 years, and the hospitals or departments whether to implement the electronic system of clinical pathway was executed around the departments and the hospitals or not.5.There were significantly differences between the way of obtaining knowledge of clinical pathways and expecting to get knowledge of clinical pathway for the clinicians.Conclusion1.Among all the scores of clinical pathway knowledge, attitude and practice for the four tertiary hospitals in Kaifeng, the level of knowledge of clinical pathways is relatively higher, while the levels of attitude and practice of clinical pathways are lower.2.Clinicians’ clinical pathways knowledge, attitudes and practice are positively related.3.The main factors on impacting clinician doctors’ clinical pathways KAP are related to departments, working years and whether implementing electronic clinical pathway system.4.There were significantly differences between the way of obtaining knowledge of clinical pathways and expecting to get knowledge of clinical pathway for the clinicians.
