Effect Observation Of The Nutritional Support In Elderly Patients With Malnutrition

Objective: The purpose of this study was to investigate the old patients with malnutrition before and after nutritional support’s pre-albumin(PA), albumin(ALB), total protein(TP), cholinesterase(CHE) levels, and the change of human body measurement to observe nutrition support effect. Through dietary survey and nutrition computing software to evaluate patients’ dietary intake, and to understand the old patients’ malnutrition dietary structure, eating habits, so as to provide more scientific and reasonable personalized dietary guidance for elderly patients.Method: Screening and researching new hospitalized elderly patients from November 2014 to January 2015 in internal medicine(respiratory, neurology and special hospital ward) of the first affiliated hospital of Dalian medical university. Assess the nutritional status by NRS-2002 during admission. Patients with NRS-2002≥3 can be divided into two groups: Receiving nutrition supporters as the nutrition intervention group, no receive nutrition supporters as the non intervention group,89 cases of elderly malnutrition patients were included in this study. Collect patients’ dietary survey, the human body measurement and laboratory data at the same time, extracting emptiness venous serum within 24 hours after admission, admission for 10 days respectively, detecting the content of pre-albumin, albumin, total protein, cholinesterase levels. Using SPSS17.0 statistical analysis, the qualitative data using frequency and rate to do the statistical description, differences between groups using chi-square test; Q uantitative data using statistical description of mean ± standard deviation, comparison between groups using independent sample t-test, inter-group comparing paired samples t test, if P<0.05, then the difference was statistically significant.Result: A total of 89 cases of elderly malnutrition patients were included in this study, among them, 61 cases of male, 28 cases of female, mean age 78.19±7.13 years old. 46 cases of 89 cases accepted nutritional support treatment, accounted for 51.7%, in addition, 43 patients did not receive nutrition support, accounting for 48.3%. Two groups’ gender, age difference has no statistical significance(P>0.05). Through dietary survey, we found that the older patients had widespread energy and protein intake is lower than the C hinese nutrition society recommends reference nutrient intake in the elderly(DRIs). In intervention group and none intervention group, after patients admitted to hospital for 24 hours, the biochemical examination, the human body measurement results had no statistically significant difference between groups(P>0.05). After admitted for 10 th day, the intervention groups’ grip strength, pre-albumin, cholinesterase were higher than that of the none intervention group, the differences between groups were statistically significant, the intervention group and the none intervention group had average other factors, there was no statistically significant difference between groups(P>0.05). In intervention group, compared the index level of the first day and the 10 th day: The compared to before intervention, the intervention group’s pre-albumin, cholinesterase, grip strength index’s difference was statistically significant; There was no statistically significant difference in other indicators. Compared to the index level of the intervention group’s first day and 10 th day: Compared with the first day, the pre-albumin factors in the none intervention group had difference statistically significant inter-group; The average difference of other indicators in the first day and 10 th day had no statistical significance.Conclusion: Pre-albumin and hand grip strength can be used as good indicator for screening elderly patients’ early malnutrition, pre-albumin can also as indicators to early judge whether the nutritional support is effective. To early assess the nutritional status in elderly patients with malnutrition and give reasonable nutrition support is effective for correcting malnutrition. And now, the rate of nutritional support in malnourished elderly patients is low, it is necessary to strengthen the doctor-patient communication, and proceed the positive nutrition support earlier.
