Abstract Objective: To investigate the diagnostic values of DCE-MRIquantitative evaluation of sacroiliac joints in ankylosing spondylitis.Methods: Forty two patients (male36, female6) with ankylosingspondylitis were selected and evaluated with the blood sedimentation,c-reactive protein and BASDAI (ankylosing spondylitis activity index).According to the activity index, we divided the patients with ankylosingspondylitis into stationary phase group and active phase group. Tworadiologists each with ten years’ experience sketched the Region of Interest(ROI) and used the extended Tofts model to determine the semi-quantitativeand quantitative hemodynamic parameters.Results: Among the42ankylosing spondylitis patients,22patients werein active phase group and the other20patients were in stationary phase group.The comparative rank sum test of active phase instensive group and stationaryphase unreinforced group were3.727、2.317、3.639、2.443、2.141、2.921andthe corresponding P value was less than0.05,which had aremarkable statistics significance. So the Ktrans、Kep、Ve、TTP、MAXConc、AUC of enhanced group/instensive group and the not enhanced group/unreinforced group had a significant difference. The index value of the enhanced group/instensive group was significant higher than the unreinforcedgroup’s. However, there were no obvious Vp and MAXSlope differencebetween enhanced and unreinforced group (P>0.05).The correlation coefficients of Ktrans、Kep、Ve、TTP and BASDAI were0.714、0.430、0.676、0.536and the corresponding P value was less than0.05,which had a remarkable statistics significance. It indicated that the scores andthe Ktrans、Kep、Ve、TTP were positive correlation, the higher of the Ktrans、Kep、Ve、TTP, the higher of the scores accordingly.Conclusion: DCE-MRI quantitative parameters can differentiate betweenthe active phase and stationary phase in ankylosing spondylitis. Strongcorrelations exist between hemodynamic parameters of Ktrans, Kep, Ve andBASDAI score.
