Objective:The clinical and laboratory characteristics of late onset ankylosing spondylitis(LAS)were analyzed and compared with adult-onset ankylosing spondylitis(AAS),ankylosing spondylitis whose diagnosis is delayed.To investigate the characteristics of late-onset ankylosing spondylitis,increase the focus on this group and to achieve timely diagnosis,early treatment.Methods:A retrospective analysis was made among 70 cases of late-onset ankylosing spondylitis at the First Affiliated Hospital of Zhengzhou University from January2014 to January 2017.Patients’general data,clinical manifestations,laboratory testing indicators and imaging parameters were collected,and relevant data of these two groups of patients were analyzed.SPSS21.0 statistical software was used for analysis.Measured data were expressed as mean±standard deviation.The t-test was used for the measurement data between the two groups.Theχ~2test was used for the comparison between groups of count data.P<0.05 was considered statistically significant.Results:Through the analysis of 70 cases of late-onset ankylosing spondylitis,we can find that the male to female ratio of LAS is 1.41:1,and with more chronic complications.When compared with the AAS group,we can summarize some results.At the beginning of the disease,we can find that the neck(cervical spine),thoracic ribs,and sternoclavicular joint pain were more common in the late-onset group than in the adult-onset group.Theχ~2 test results showed that the P values were 0.01,0.03,and P<0.05,respectively.Buttock pain are more common in adult-onset groups,P values was 0.02.The difference was statistically significant.Low back pain,hip pain,shoulder pain,toe(inflammation),enthesitis,arthritis of the lower extremity,arthritis of the upper extremity,and fever showed that P>0.05,and the difference was not statistically significant.During the disease process,we can find that neck(cervical spine)and upper limb joints are more often damaged in the late-onset group than in the adult-onset group.Theχ~2 test results showed that the P values were 0.01,0 and P<0.05,respectively.Lumbar,ankle,hip,shoulder,toe(inflammation),enthesitis,arthritis of the lower extremity,rib cage,sternoclavicular joint,and extra-articular performance,theχ2 test showed that P>0.05.No statistical difference.Comparing laboratory-related variables,the late-onset group had a higher mean ESR than the adult group,with a P value of 0.002,<0.05,and the difference was statistically significant;between the two groups,CRP,peripheral white blood cells,peripheral blood platelets,and D-dimer The results of the examination of fibrinogen,HLA-B27showed a P value of>0.05,and the difference was not statistically significant.Comparing the MRI of ankle joints,it showed that hip inflammation was more common in the adult-onset group than the late-onset group,with a P value of 0.003,less than 0.05,and the difference was statistically significant;hip effusion,greater trochanter attachment inflammation was no significant difference(P>0.05).Conclusion:Late-onset AS is characterized by significantly increased laboratory measurements of inflammation,and has a higher frequency of upper-limb joint and cervical involvement than adult-onset AS.
