Objective:To explore the clinical efficacy of heat-clearing and detoxicating andactivating blood therapy for postoperative wound-surface healing of perianal or perirectalabscess at the early stage,and study the mechanism of it.Methods:Compound Huangbai solution as a representative of heat-clearing anddetoxicating and activating blood, has been widely used in clinical, perianal or perirectalabscess on early postoperative dressing achieved good clinical efficacy. To chose60patients with this disease who underwent surgery clinically,and randomized them intocompound Huangbai group and vaseline group,30cases for each one.From the second dayafter surgery, based on the seem conventional treatment pre-postoperatively,the patients ofthe first group were given dressing change with compound Huangbai solution gauze,whilethe second group were given with vaseline gauze,one time per day. Then the postoperativehealing time of wound surface, the healing rate at different stage,the situation ofgranulation growth and the pain degree of patients were observed and analyzed statistically.The experimental study is made of full thickness skin defect wound inoculation of Wistarrat model,some wound were injected with staphylococcus aureus and escherichia coli tomake infectious wound,which were observed to find out the skin regrowth state innormal group, model group, Chinese medicine group and Western medicine, and detectthe expression of wounded tissue pathological changes of TNF-α and IL-6.Results: The wound-surface healing time of the first group is obviously shorter thanthat of the second group,and without abnormal situations were found in each observational index at the3rd day after surgery,but the healing rate,the situation of granulation growthand the pain degree of the first group were superior to that of the second group at the7thd(P<0.05),and the compound Huangbai group is superior to vaseline group in healing rateand the situation of granulation growth at the14thday(P<0.05),and there was no statisticaldifference in pain degree between both groups. Lab studies show that compound Huangbaisolution could lower the expression of TNF-α、 IL-6protein level for infectiouswound-surface,then to reach the purpose of controlling the inflammatory degree of thewound-surface,improving the environment for the epidermal stem cell proliferation andmigration,and shortening the wound-surface healing time,thus to promote thewound-surface healing.Conclusion: The main postoperative pathogenesis of perianal or perirectal abscess atthe early stage is heat-toxin brimming and collaterals obstruction by blood stasis,and thistherapy can obviously promote the postoperative wound-surface healing of perianal orperirectal abscess.
