BackgroundAcne is a chronic inflammatory skin disease of the pilo-sebaceous unit, which mainly occurs in adolescents. The lesions maymanifest as polymorphous skin lesions, such as facial comedones, papules,pustules, nodules and so on. The inflammation often subsides with thepigmentation, persistence erythema, atrophic or hypertrophic scar. Italways affects the beauty of patients, influences the patients with differentkinds of psychological pressure, and even affects their education andemployment. So people pay more attention to acne nowadays. Thepathogenesis of acne is multiple, which has been manifested mainlyrelating to the levels of sex hormones, irregular sebaceous glands control,abnormal keratinized sebaceous glands, microorganism in the hair follicle,inflammation and immune response. In order to prevent or treat acne, wecan block these relating steps.Fruit acid is a unified call of a series of carboxylic acid with hydroxylin α-position. A lot of studies have confirmed that the fruit acid canweaken cell adhesion of keratinocyte, reduce the accumulation of keratin,activate steroid sulfatases and serine proteases to degrade desmosomes.Besides, it also can eliminate the death cells in the opening of the sebaceous gland, form a layer of natural protective screen, protect theinner skin and prevent the intrusion of bacteria. Furthermore, fruit acid canincrease the thickness of dermal papilla layer and density of elastic fiber,resulting the increase thickness and elasticity of skin. What’s more, it caninhibit the activity of tyrosinase and release hyaluronic acid that acts as askin moisturizer. Because of the above characteristics of fruit acid, it hasbeen of great concern to the medical profession and the beauty industry. Itis not only used in treatment of acne, chloasma, the light aging,keratinizing diseases, but also used for the skin beauty.ObjectiveObserve the efficacy and safety of Alpha Hydroxy Acid in thetreatment of mild-to-moderate acne and evaluate the improvement of skintexture and color through questionnaires.MethodsChoose22patients with mild to moderate acne according to theselection criteria. Alpha Hydroxy Acid with concentration of20%,35%,50%or70%was applied to treat the patients. Operation methods are asfollow: Firstly, we ask the patient to lie down after regularly cleaning facewith water, then we wrap the hair of the patient with a towel. Secondly,paint skin protection cream in the corner of the eyes, the groove of theskin and the places with previous wound in order to protect the skin. Thenprotect the eyes with3-4or more layers of wet gauze. Thirdly, brush the acid from the forehead to the operation area quickly, stay3-5minutes.Fourthly, according to the reaction, spray neutralization solutioncompletely to no bubbles. Finally, remove protective cream for protectingskin lesions and gauge, apply the soothing moisturizing silky mask on facein15-20minutes. Usually we take photos and taking notes before eachtreatment and after three weeks of the last treatment. We will recordchanges in patients with inflammatory damage or adverse reactions andevaluate the improvement of skin texture and color through thequestionnaire.Results22patients were enrolled. After two courses, the inflammatorylesions subsided rate is58.7%. The inflammatory skin lesions subsidedbetter than non-inflammatory skin lesions. After three courses, theinflammatory lesions subsided rate is83.6%. the non-Inflammatorylesions subsided rate is73.6%, both of them were reduced. The totalremission rates of lesions of the four treatments were20%,50.6%,77.2%,90.2%. With the times of treatment increased, the total regression oflesions was gradually increased, the difference was statisticallysignificant(P<0.01). Inflammatory lesions subsided rate was higher thanthat of non-inflammatory lesions(P<0.01).In22patients, the totaleffective rate was90.9%with40.9%cure rate,50%effective rate. Thetotal effective rates of4treatments were13.6%,31.8%,63.6%,90.9%. The total effective rate increased with the increase of the treatment times.ConclusionsTreatment for acne with Alpha Hydroxy Acid has the followingadvantages.(1)High effective rateThey are effective for inflammatory damage and non-inflammatorydamage.(2)Highly safeAlpha Hydroxy Acid is a kind of natural organic acid. No sideeffects will happen when the concentration reached70%(3)Simple and easyAcid skin is from low concentration to high concentration gradually.Change a violent exfoliation is into several gentle exfoliation.
