Objective:(1) To explore the etiology and pathogenesis and treatment of Tic Disorder to provide a basis for clinical medicine through the literature theory and clinical research of traditional Chinese medicine and Western Medicine on Tic Disorder.(2) To study the correlation between Tic Disorder and anaphylax preliminarily, to try to provide the support of objective indicators in the disease pathogenesis.(3) To observe the clinical curative effect of Qufeng antispasmodic in treating Transient Tic Disorder (Syndrome of lung wind and liver hyperactivity).Method:(1)Part of literature study:To sort and analyze the collection of relevant literatures between Chinese Medicine and Western Medicine in Tic Disorder, To form an understanding of the overall treatment in Tic Disorder (Syndrome of lung wind and liver hyperactivity).(2) Part of allergy related research:63patients at the age of2-15years old, diagnosed with Tic Disorder were in the observation group,200cases of Non Tic Disorder at the age of2-15years old (excluding Tic Disorder, ADHD and OCD in children), were the non tic disorder group. Allergen positive rate between the two groups had been compared. At the same time, different syndromes one the base of TCM were classified in the observation group.(3) Part of clinical research:60patients at the age of2-15years old, diagnosed Transient Tic Disorder, belonging to the Syndrome of lung wind and liver hyperactivity in TCM were taken as the object of study. They were divided into treatment group and control group with30cases in each. The patients in treatment group had been given oral medicine decoction of Qufeng antispasmodic, while that in the control group had been given western medicine of Tiapride Hydrochloride Tablets, Inosine Tablets and Lysine, Inosite and Vitamin B12Oral Solution. The treatment lost for12weeks, and follow-up visit continued for4weeks after the treatment. The changes of clinical symptoms and credits in two groups were observed and recorded before and after the treatment. Finally the obtained data were statistically processed and analyzed to objectively evaluate its clinical curative effect.Results:(1) The result of literature study:The modern scholars attributed Tic Disease to the category of" febrile disease with symptoms such as convulsion, opisthotonos, trismus "," liver wind "," convulsions", and "chronic infantile convulsion" etc, which is close to the relationship of the liver and wind. Tic Disorder was also related to the factors of the congenital deficiency, emotional disorders, improper diet and being invaded by exogenous pathogen, the five excessive emotional activities, and internal accumulation of wind and phlegm. In all treatment calming the liver to stop the wind was the basic principles. All the doctors differentiated the sorts of the diseases and applied different prescriptions according to the etiology, pathogenesis and syndrome of tic disorder and all treatments had their characteristics. Allergic considerations may be an important cause of tic disorder. The treatment of tic disorder from the lung often can achieve a good effect.(2) The results of allergy related research:The allergen skin prick test observation from the children with tic disorder and non tic disorder showed:Allergen positive rate was P<0.05. The allergens positive rate in observation group was higher than that in non tic disorder group. In an intra-group comparison in the observation group we found that the Tic Disorder allergen positive rate in TTD, CCT, TS, the three types had no difference (P>0.05). The TCM syndrome of Tic Disorder was the syndrome of lung wind and liver hyperactivity. The frequency number appeared58cases, covering the ratio of92.1%.(3) The results of clinical research:The total effective rate of treatment group was93.33%, and the total effective rate of control group was86.67%. The motor tic score, vocal tic score, tic score and cough, stuffy nose, throat itching, throat phlegm symptoms in traditional Chinese medicine in the treatment group and the control group in the2,4,6,8,10,12weeks indicated a statistical significance (P<0.05). The significant curative effect in the treatment group and the control group was presented in the treatment of children with TTD. The Comparison of the general curative effect in the two groups was P>0.05, which showed that there was no obvious difference in the treatment group and the control group. Chinese medicine treatment group and Western medicine control group reached nearly the same in curative effect. After analyzing each main symptoms and the accompanying symptoms we found that the main symptoms including blink of an eye, shrug nasal, cough, pharyngeal itching, throat phlegm nasal obstruction in the treatment group were better improved than that in the control group:(P<0.05). In the follow-up visit after treatment, no recurrence was discovered in the two groups.Conclusion:(1) Each of the doctors differentiated the sorts of the diseases and applied different prescriptions according to the etiology, pathogenesis and syndrome of tic disorder and all treatments had their own characteristics. The treatment of tic disorder from the lung often can achieve a good effect.(2) Allergy may be one of the causes for tic disorder.(3) The Qufeng antispasmodic is one of the effective prescription in treating TTD, and its curative effect in clinical treatment was positive. It is worthy of development.
