Objective: Conduct a research on the safety and effectiveness for Gangdi Pingchuandecoction in the treatment of chronic obstructive pulmonary disease in acuteexacerbation (AECOPD).Methods:79patients conforms to the standard diagnosis of acute exacerbation ofchronic obstructive pulmonary disease and lung distension (phlegm and blood stasisof TCM Syndrome Differentiation Standard) were randomly divided into treatmentgroup and control group,39cases in the treatment group,40cases of the controlgroup, two group are using routine treatment, Patients in treatment group were treatedwith convention therapeutic schedule and daily Gangdi Pingchuan decoction enema,treatments for1week, Observation the changes of TCM symptom scores, pulmonaryfunction, arterial blood gas analysis and safety indicators before and after treatment.Results:1.TCM syndrome total effective rate of treatment group was higher than thatof the control group, the treatment group was97.44%,75%in the control group,significant difference between the two groups was significant (P<0.01). the treatmentgroup in expectoration, shortness of breath, wheezing, anorexia and bloating,constipation and other improved significantly better than the control group (P<0.01).Two groups were improved, cough, sputum, wheezing, shortness of breath, chesttightness, dry and wet rales and other symptoms, there is significant difference beforeand after treatment (P<0.05), to improve constipation, anorexia and bloating, thetreatment group had significant difference (P<0.05), while the control group showedno significant difference (P>0.05), two groups of cyanosis was not significantlyimproved (P>0.05).2.In the improvement of pulmonary function, compared before and aftertreatment, FVC and FVC%were statistically significant (P<0.05), the control groupimproved pulmonary function showed no significant difference (P>0.05).3.In the anti-infection, infection control effect of treatment group was superior tothe control group, the white blood cell count (WBC) and the percentage of neutrophils (NE%) decrease, lymphocyte ratio (LY%) increased relatively, there was statisticallysignificant difference (P<0.05). Comparison of curative effect on WBC, NE%, LY%were significantly different in the two groups (P<0.05).4.In the improvement of blood gas analysis, the treatment group in increase ofPO2and SaO2, reduce of PCO2was more significantly, there was statisticallydifference (P<0.05). The treatment group PO2, PCO2, SaO2group, a statisticallysignificant difference (P<0.01); the PO2in control group were statistically significantdifference (P<0.05), while the PCO2and SaO2group were not statisticallysignificance (P>0.05).5.In the security index, no significant difference between the treatmentgroup before and after treatment with ALT, AST, BUN, CR (P>0.05).Conclusion:1.Gangdi Pingchuan decoction enema can improve TCM syndrome,anti-infection, pulmonary ventilation function, alleviate hypoxia and carbon dioxideretention, confirmed this decoction can improve the the therapeutic effect ofAECOPD, treatment of integrated traditional Chinese and Western Medicine issuperior to conventional therapy.2.Using removing phlegm and blood stasis, urgent Tongfu enema method forAECOPD is a more effective treatment.
