Objective: To observe the clinical efficacy and safety of Qingfei Pingchuan Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease with Phlegm-heat depressing Lung syndrome. Methods:This study included 60 patients with acute exacerbation of chronic obstructive pulmonary disease and TCM syndrome differentiation of phlegm and heat syndrome.According to the random number table,They were randomly divided into treatment group and control group with 30 cases each. The control group was treated with antibiotics, bronchodilators and glucocorticoids on the basis of general treatment; The treatment group added Qingfei Pingchuan tang of Chinese medicine treatment on the basis of the control group, the daily dose,3 times a day.The two groups of patients were 10 days of treatment. After the end of the treatment, the changes of TCM syndrome scores, blood routine, peripheral oxygen saturation, lung function, and chronic obstructive pulmonary assessment test(CAT)were statistically analyzed to evaluate the clinical efficacy.Results:1.There were no statistically significant differences in gender, age and disease course between the treatment group and the control group before treatment, and the two groups were comparable(P >0.05).2.The total effective rate of TCM syndromes in the treatment group was 93.33%,and the total effective rate in the control group was 83.33%.The total effective rate of TCM syndromes in the treatment group was higher than that in the control group, andthe difference between the two groups was statistically significant(P<0.05).3.Both the treatment group and the control group could improve the TCM syndrome scores of the patients. The treatment group was superior to the control group, and the difference was statistically significant(P<0.01).The treatment group improved the patients with wheezing, cough, sputum, sputum, thirst and the thirst was better than the control group, the difference was statistically significantt(P<0.05).There was no significant difference in chest tightness, chest pain and fever between the treatment group and the control group(P>0.05).There was no significant improvement in stool abnormalities in the control group, and the difference was not statistically significant(P>0.05).4.Both the treatment group and the control group could improve the lung function of the patients, the treatment group was superior to the control group, and the difference was statistically significantt(P<0.05).5.The treatment group can reduce white blood cell count, neutrophil percentage,procalcitonin(P<0.01),and the control group can reduce white blood cell count(P<0.05),neutrophil percentage,procalcitonin(P<0.01).The treatment group decreased the white blood cell count and the percentage of neutrophils better than the control group, the difference was statistically significant(P<0.05);the degree of reduction of procalcitonin was similar in the two groups, the difference was not statistically significant(P>0.05).6.Both the treatment group and the control group could improve the oxygen saturation of the patients(P<0.01).The improvement of the two groups was similar, and the difference was not statistically significant(P>0.05).7.Both the treatment group and the control group could improve the distribution of CAT score and CAT score(P<0.01).The improvement of the two groups was comparable, and the difference was not statistically significant(P>0.05).8.There were no significant fluctuations in liver and kidney function, urine routine, stool routine, and electrocardiogram before and after treatment in the treatment group and the control group, the difference was not statistically significant(P>0.05),and the patient did not have adverse reactions. Conclusion:This study shows that the combination of Qingfei Pingchuan Decoction in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease(Phlegm-heat depressing Lung)is more effective than Western medicine alone. It is superior to the control group in improving the TCM symptoms, inflammatory indexes, and lung function of patients, and has no obvious side effects. |
