Stress Mechanism Study Of Adult Femoral Head Necrosis

Objective:By reviewing the imaging data of adult femoral head necrosis, analysing the stress distribution characteristics of the hip joint in daily life, and the characteristics of the blood supply of femoral head. Make a statistical analysis of the correlation between the femoral head necrosis area and the stress peak area, morever, the correlation of blood supply area, put forward the stress mechanism of femoral head necrosis. Emphasizes the importance of the stress factors in the process of the occurrence and development of femoral head necrosis. Put the therapy of nourishing the liver and kidney as well as strengthening tendons and bones on the equally important position, to the therapy of improving the blood circulation. So as to better guide the prevention and treatment of femoral head necrosis in clinic.Methods:Collecting patients data from the first affiliated hospital of guangzhou university of TCM in April 2014 to April 2015 diagnosed with femoral head necrosis with hip spiral CT image data, sum to 23 patients with 40 hips,15 cases were male,8 cases were female; Age 18 to 47, with an average age of 33 years old; ARCO phases:phase I with 2 hips, phase II with 16 hips, phase III with 21 hips, phase IV with 1 hip; Causes:wound necrosis 5 hips, hormone necrosis 18 hips, alcohol necrosis 14 hips, idiopathic necrosis 3 hips. Use Mimics software to reconstruct 3d images, determining the femoral head necrosis area and center of the necrosis area. According to the characteristics of the femoral head stress in daily life, we divided the femoral head into the top column, the lateral column, the front column and the back of four areas, among which the first three columns are the stress concentration area in daily life, while the back column is the blood supply concentration area of the femoral head. Set the femoral head midperpendicular around 40 degrees range is the top column, its lateral is the lateral column, its front and back side are the front column and the back column [1]. Match each necrosis center of the femoral head to the column area, record the data, establish a database, and make statistical analysis.Results:23 cases of the hip spiral CT image data (40 hips) were successfully for 3d reconstruction, the necrotic center located in the needle with 4 hips, located in the side column were 8 hips, located in the lateral column of the femoral head were 26 hips, located in the back column were 2 hips, after the single sample chi-square (χ2=1.11, P<0.05), the difference was statistically significant.Conclusion:(1) In the daily life, stress concentration area is in the needle and anterolateral of the femoral head, femoral head necrosis and collapse are mainly concentrated in the region;(2) The blood supply of femoral head are mainly distributed behind the femoral head, femoral head necrosis and collapse rarely occurred in the region, necrosis and blood supply distribution does not match;...
