ObjectiveThrough using Mimics software to systematic review the imaging information of osteonecrosis of the femoral head, which included of anatomy, blood supply and stress distribution of daily life in the hip and femoral head, we try to local osteonecrosis of the femoral head and statistical analysis the peak of stress areas distribution respectively. We try to verify the correlation between them and theorize the biomechanical mechanism of the osteonecrosis of the femoral head. We want to emphasize the mechanism play an important role in development of osteonecrosis of the femoral head. As a result, it can provide effective guide treatment of osteonecrosis of the femoral head in clinical.MethodsData were collected from paitents of osteonecrosis of the femoral head, who diagnosed in our hospital from September 2011 to March 2016. According to the inclusion criteria and exclusion criteria, it includes of a total of 56 patients (56 hips). Though using Mimics software to import and convert the data,we reconstruct three-dimensional images of patients’hip, as well as the area of necrosis and corresponding of the center point of necrosis. Then though simulating the standing, walking and seating position, the author try to analysis the peak of stress areas distribution of the three positions. The resulting data will be established a database, statistical methods were analysised.ResultsThe information of patients:29 males and 27 females;mean age 45 years years old(18-83). The range of the osteonecrosis including of I level 7 hips, II level 14 hips, III level 17 hips, IV level 18 hips. The osteonecrosis distribution:30 cases in the anterior column,2 cases in the outside column,2 cases in the top column and 22 cases in the top-outside column. The peak of stress areas distribution:in the standing position,27 cases in the front column,2 cases in the posterior column,3 cases in the top column,24 cases in the top-outside columns; in the seating position,20 cases in the front column,3 cases in the posterior column,12 cases in the top column, 21 cases in the top-outside column;in the walking position,28 cases in the front column,3 cases in the posterior column,2 cases in the top column,23 cases in the top-outside columns.There was significant difference between the range of osteonecrosis of the femoral head and the peak of stress areas distribution in different positions (P value= 0.025), the different ranges of osteonecrosis of the femoral heads and the peak of stress areas distribution (P value= 0.972). In the standing and walking position,the distribution of osteonecrosis of the femoral head differs the peak of stress areas distribution P values>0.05.ConclusionThe different ranges of osteonecrosis of the femoral head determine the difference of the peak of stress areas distribution. However, The necrotic area is greater, the peak of stress areas smaller. And it is the maximum of the peak of stress area in the walking position, followed by sitting position, the minimum of the standing position. In the standing and walking position, the peak of stress areas distribution is consistent with the distribution of osteonecrosis of the femoral head, mainly in the front columns and top-outside columns of the femoral head.
