Purpose There are many kind of measurement methods evaluation the extent of the osteonecrosis of femoral head during early stage in adulthood,for its MRI images,Kim method,Koo necrosis index method,Cherian come up with the modified necrosis index method,Hernigou method,the direct measurement method use the MIMICS,etc.,but the accuracy have been reported differences,the standard are different,too.And the most important problem is to predicted the femoral head when to collapse.there are not a widely recognized from Doctors.In this research we try to compare the Kim method,the Koo method,the Cherian method,the Hernigou method,the direct measurement method by MIMICS,and try to find the relationship among them?Methods To select the patients diagnosed in Affiliated Zhongshan Hospital Of Dalian University in 2016-2017,48 cases with 51 hips in ARCO ?.Measure these MRI images with Kim's method,Koo method,Cherian method,Hernigou method,and the MIMICS method.Try to find connections between these methods,looking for their relationship with the ARCO stage.Results Measuring ARCO stage ? patients(male 37 female 11)with their MRI images,Hernigou method showed the necrosis volume was 10458.61±8567.03 mm3,the entire volume of femoral head was 43240.29±9221.16 mm3,MIMICS measurement get the damaged parts volume10428.73±8571.76mm3,for the entire femoral head the average volume of 43319.29±9213.43 mm3,there's no statistical difference between them.Kim method(46.16 + + 16.80)and Koo method(46.25 + + 21.30)are not statistically difference: t=-0.037,P(Sig.)=0.971.Kim method 47.30(28.65 ~ 62.75)and Cherian method49.00(33.05 ~ 85.90): Z=-2.062,P=0.039<0.05,the difference are statistically significant.Koo method(46.25 + 21.30)and the Cherian method(56.19 + 26.93)are statistically difference: t=-4.161,P(Sig.)=0.01(P<0.05).Kim method(46.16 + 16.80)and MIMICS method(22.95 + + 17.21)are statistically difference: t=7.412,P(Sig.)=0.00(P<0.05).The Koo method(46.25 + + 21.30)and the MIMICS method(22.95 + + 17.21)are statistically differences: t=8.099,P(Sig.)=0.00(P<0.05).The Cherian method(56.19 + 26.93)and the MIMICS method(22.95 + + 17.21)are statistically differences:t=9.446,P(Sig.)=0.00(P<0.05).The Spearman test was conducted in Kim method,Koo method and Cherian method and the MIMICS method: Kim method:(Rs=0.753,P=0.03),Koo method:(Rs=0.852,P=0.00);Cherian method :(Rs=0.874,P=0.00),P <0.05 they all statistically difference.Conclusions Hernigou method and three-dimensional reconstruction method use MIMICS can accurately reflect the extent of the osteonecrosis of femoral head.The data measured by the Kim method,the Koo method,and the Cherian method cannot be equated with the lesion volume ratio of the osteonecrosis of femoral head,and there is no corresponding relationship between them.But the Kim method,the Koo method,and the Cherian method have a positive correlation with the three-dimensional reconstruction method.The Cherian method is more accurate than the Kim method and the Koo method.And the Kim method and the Koo method,the Cherian's modified index method,Hernigou method,three-dimensional reconstruction method,these kinds of measurement methods can't correspond with ARCO stages,and directly use the data to correspond to the ARCO stage,making the staging period larger than it actually is,there is a clear tendency to increase of the assessment of the patient's condition.
