Arthroscopy Combined With Involving The Bone Mixture Clinical Research For The Treatment Of Knee Osteoarthritis

Objective: To observe the joint arthroscopy surgery involving the bone mixture of knee osteoarthritis clinical efficacy, the patient’s symptoms and physical signs before and after treatment was observed and measured before and after treatment in patients with joint fluid and peripheral blood of IL- 6, TNF, MMP- 3 numerical analysis of mechanism of its effect in improving symptoms and to explore the clinical advantages of combination of Chinese and western medicine therapy on knee osteoarthritis.Methods: Methods: clinical choose 60 cases of patients with knee osteoarthritis, all cases were from September 2013 to November 2014, the first affiliated hospital of yunnan institute of orthopaedic outpatient and inpatient. According to the principle of random grouping 60 patients were divided into traditional Chinese medicine treatment group, arthroscopy cleaning technique and the combination of Chinese and western treatment group, each group 20 cases respectively.(involving the bone mixture) of traditional Chinese medicine treatment group used internally. Arthroscopy group only by making clear pure arthroscopy cleaning technique. Combination of Chinese and western treatment group cleaning operation under the arthroscope treatment, based on the internal and traditional Chinese medicine for treatment. After treatment were observed before and after treatment in patients with signs and symptoms improve the situation, the determination of before and after treatment of the patient’s joint fluid and peripheral blood IL- 6, TNF, MMP- 3 values, using SPSS statistical analysis software, evaluate curative effect before and after contrast.Results:(1) the three groups of cases before and after treatment(P < 0.01), suggesting that three methods of treatment for knee osteoarthritis(liver and kidney deficiency, relative stasis type) has good curative effect.(2) of three groups of patients with pain, walking and daily activity of the integral in each group before and after treatment had significant difference(P < 0.001), and shall remain valid for the treatment of knee osteoarthritis, arthroscopy and traditional Chinese medicinetreatment group effect is best, followed by arthroscopy group, is Chinese traditional medicine group again.(3) the three groups after treatment of three kinds of cytokines in the blood(IL- 6, TNF alpha, MMP- 3) was in both the level of average reduced in different degrees(P < 0.01), on the IL- 6, MMPS index, combining Chinese and western medicine group difference before and after the treatment effect is better than the other two groups, with significant difference(P < 0.05); Chinese traditional medicine group the difference before and after the treatment effect and arthroscopy, comparison between the two there was no significant difference(P > 0.05); On the index of TNF alpha, three groups had no significant difference(P > 0.05).(4) three groups after treatment of three kinds of cytokines in joint fluid(IL- 6, TNF alpha, MMP- 3) was in both the level of average reduced in different degrees(P < 0.01), the index of IL- 6, arthroscopy difference than the traditional Chinese medicine(TCM) before and after the treatment group effect is good, with significant difference(P < 0.05); Difference before and after the combination of Chinese and western medicine treatment group better than arthroscopic group, but the difference is not significant(P > 0.05). On TNF alpha, MMP- 3 indicators, arthroscopy difference before and after the treatment group and combine traditional Chinese and western medicine group were better than effect of traditional Chinese medicine group, with significant difference(P < 0.05), combining Chinese and western medicine group with arthroscopy group had no significant difference(P > 0.05).Conclusion: the combination of the arthroscopic cleaning operation involving the bone mixture has a better internal and curative effect on knee joint osteoarthritis, can alleviate the symptoms and the knee joint function, decrease related inflammatory cytokines(IL- 6, TNF alpha, MMP- 3), and delay the onset of osteoarthritis of knee joint development, improve the quality of life of patients.
