Objective: by comparing and observing the clinical effect of traditional Chinese medicine bone-setting therapy, oral diclofenac sodium sustained release tablets and intra-articular injection of sodium hyaluronate on knee osteoarthritis(KOA),scientifically and objectively analyze the clinical value of two kinds of treatment methods, further explore the mechanism of action of traditional Chinese medicine bone-setting therapy,to provide clinical basis for the application of traditional Chinese medicine bone-setting therapy in the treatment of KOA,in order to better guide clinical work. Methods: according to the inclusion criteria,96 patients with knee osteoarthritis who were admitted to the outpatient department of orthopedic injuries, affiliated hospital of Hebei college of traditional Chinese medicine between January 2017 and June 2018 were randomly divided into the experimental group and the control group with IBM SPSS Statistics 21.0software according to the serial number of visits(among them,2 cases in the experimental group were excluded due to self-administered oral analgesics, and 1 case was lost to follow-up;3 cases in the control group were lost to follow-up.Finally,there were 45 cases in the experimental group and 45 cases in the control group). The age, gender, course of disease, and k-l radiological grading of the knee joint of the two groups were compared, showing no statistically significant difference in general data between the two groups(P>0.05),indicating that the two groups were comparable; The experimental group was treated with traditional Chinese medicine bone-setting therapy(fixed-point rotatory reduction of spine, pelvic correction and knee bone-setting manipulation)twice a week for five times as one course of treatment;the control group was treated with intra-articular injection of sodium hyaluronate, diclofenac sodium sustained release tablets, once a week for fivetimes as one course of treatment. In addition, both groups received health guidance and functional training.VAS score and WOMAC osteoarthritis scale were filled in before, after and 3 months after treatment, and the total clinical effective rate was calculated according to WOMAC score 3 months after treatment; The adverse reaction record form was filled at the end of the treatment. Results: by comparing the VAS score and WOMAC scale score(P>0.05)before treatment between the two groups,it was found that there was no statistical significance and there was comparability.VAS score and WOMAC score of the two groups were significantly improved after treatment and 3months after treatment compared with those before treatment(P<0.05),indicating that both the experimental group and the control group could reduce the symptoms of knee joint pain; There was no significant difference in VAS scores between the two groups after treatment(P>0.05).After treatment, there was a statistically significant difference in WOMAC score between the two groups(P<0.05),and the experimental group was superior to the control group.VAS score and WOMAC score in the experimental group were significantly improved 3 months after treatment(P<0.05).The VAS score and WOMAC score of the two groups at 3 months after treatment showed statistically significant difference(P<0.05),and the experimental group was superior to the control group.Comparison of clinical efficacy: the total effective rate of the experimental group was 93.3%;The total effective rate in the control group was 84.4%.The difference of total effective rate between the two groups was statistically significant(P<0.05),and the experimental group was superior to the control group.During the treatment and follow-up,no serious adverse events occurred in the patients of the two groups, and no obvious abnormalities were observed in body temperature, respiration, pulse, blood pressure, blood routine, electrocardiogram and liver and kidney functions before and after the treatment, and no other allergies or adverse reactions were observed. Conclusion: both traditional Chinese medicine bone-setting therapy andintra-articular injection of sodium hyaluronate have good short-term effects on knee osteoarthritis, but the former is significantly better than the latter in alleviating stiffness and limiting joint function of patients, and the long-term effect has a significant advantage. In a word,f has achieved the purpose of this project through clinical research, that is,to clarify the clinical efficacy of traditional Chinese medicine bone-setting therapy in the treatment of knee osteoarthritis, thus providing clinical basis and reference value for the application of traditional Chinese medicine bone-setting therapy in the treatment of KOA.
