| Objective: The study aim to propose a effective and convenient therapy as well as affordable in prices on the treatment of lumbar disc herniation with Cold Condensation type, to ease the pain of lumbar disc herniation and improve patient’s life quality. To observe the score of visual analogue scale(VAS) before and after treatment,the clinical effect evaluation of <Guiding Principle of clinical research on New Chinese Traditional Medicine> and JOA scores, in order to provide theoretical basis and investigate the action mechanism of acupuncture of “Tiao Du Li Jin” combined with “Hanjing”decoction on lumbar disc herniation with cold condensation TypeMethods: Study objects : 120 outpatients and inpatients from department of orthopedics who conform to the uniform clinical diagnostic criteria, inclusion criteria and exclusion criteria were randomly divided into acupuncture combined with medicine group,acupuncture group and chinese medicine group,40 patients in each group. All of 120 patients in our study came from Hebei Pronvince of Traditional Chinese Medicine between January,2013 and December,2014.Experimental methods:Acupuncture combined with medicine group:40 patients in this group received the treatment of acupuncture combined with“Hanjing” decoction.1. Acupuncture of “Tiao Du Li Jin” ①Point locating: yaoyangguan, mingmen, yaojiaji,houxi,dachangshu,shenshu,huantiao,yanglingquan. ② Operating method: After routine disinfection, the point of huantiao received triple needling, the point of yaoyangguan, migmen, yaojiaji, dachangshu, shenshu,houxi and yanglingquan received routine needling. Time of needle retaining was 30 minutes, once time per day for two weeks.2. Oral administration of “Hanjing” decoction:①Component:“Hanjing” decoction was made up of “Guiganjiangzaomaxinfu”decoction, Wugong and Quanxie. The type of kidney-yang deficiency added “Erxian” decoction(Xianmao 12 g, Xianlingpi 12g).The type of kidney-yin deficiency added Shudi 15 g,Shanzhuyu 15 g.The type of qi deficiency added Hongshen 10 g.The type of blood stasis syndrome added Tiaoren 10 g,Honghua 10 g. The type of damp syndrome added Baizhu 10 g.Once time per day for two weeks.Acupuncture group: only acupuncture, the needling methods as same as acupuncture combined with medicine group.Chinese medicine group: only oral administration of “Hanjing” decoction. Administration and dose of “Hanjing” decoction as same as acupuncture combined with medicine group.Health guidance: ① To avoid or reduce waist weight bearing; ② To strengthen the protection of lumbar:adorn sash;③To take moderate waist function cxercise in bed.Result:1 There were no abnormal results of ECG, blood routine, urine routine, hepatic and renal function, four projects of blood coagulation. There were no significant changes of above-mentioned tests before and after treatment.2 Clinical effects comparison:In acupuncture combined with medicine group, 20,10,8,2 patients were cured, markedly improved, effected and failed, respectively. The total recovery rate was 98%.In acupuncture group,18,10,9,3 patients were cured, markedly improved, effected and failed, respectively. The total recovery rate was 90%. In chinese medicine group, 19,11,8,2 patients were cured, markedly improved, effected and failed, respectively. The total recovery rate was 87.5%.There were significant difference in 3 groups after treatment(Z=7.267, P=0.026).With LSD method, the clinical effect of acupuncture combined with medicine group was better than that in acupuncture group(P=0.025).The clinical effect of acupuncture combined with medicine group was better than that in chinese medicine group(P=0.014).There were no significant difference in clinical effects between acupuncture group and chinese medicine group(P=0.836).3 JOA comparison: intra-group comparison: The score of JOA after treatment were obviously increased in 3 groups compared with before treatment(P<0.001). The score of JOA increased from 10(6.25, 11.75) to 20(16,24), 9(7,11) to 17.5(12,23) and 9(6.25,11) to 17.5(13,22) in acupuncture combined with medicine group, acupuncture group and chinese medicine group, respectively. Group comparison:(1) The score of JOA were no obviously different in 3 groups before treatment(P>0.05).(2) The score of JOA were obviously different in 3 groups after treatment(P>0.01).(3)According to method of LSD, the clinical effect in acupuncture combined with medicine group was better than that in acupuncture group as well as in chinese medicine group. There were no significant difference between acupuncture group and chinese medicine group.4 VAS comparison: intra-group comparison: The score of VAS after treatment was decreased compared with before treatment in each group(P<0.001). The score of VAS decreased from 6(5,7) to 1(0,2.75), 6(5,7) to 2(1,4) and 6(5,7) to 3(1,4) in acupuncture combined with medicine group, acupuncture group and chinese medicine group, respectively. Group comparison:(1) The score of VAS were no obviously different in 3 groups before treatment(P>0.05).(2) The score of VAS showed a obvious difference in 3 groups after treatment(P>0.05).(3)According to method of VAS, the clinical effect in acupuncture combined with medicine group was better than that in acupuncture group as well as in chinese medicine group. There were significant differece between acupuncture group and chinese medicine group.Conclusions:There are good results of acupuncture of“Tiao Du Li Jin”, Oral administration of “Hanjing” decoction and acupuncture of “Tiao Du Li Jin”combined with “Hanjing”decoction on lumbar disc herniation with cold condensation Type. The clinical effects in acupuncture combined with medicine group is superior to acupuncture group as well as chinese medicine group. The improvement of VAS between acupuncture group(acupuncture of “Tiaodulijin”) and chinese medicine group(“Hanjing”decoction) is obviously different that show a distinctly analgesic effects in acupuncture of “Tiaodulijin”. |
