ObjectivesWith the transformation of medical model and the development of medicine, people gradually realize that drug treatment can’t completely meet the needs of the COPD patients. Therefore, more and more doctors and patients start to pay attention to chronic disease management. But the model of chronic disease management of TCM is not clear yet. This study is aim to investigating on the effect of increasing chronic disease management of TCM in COPD patients, developing an appropriate chronic disease management pattern and providing the basis data for clinical promotion.Methods60 stable COPD patients were randomly divided into 2 groups,30 in intensive management group and 30 in the general one. Both groups were treated with chronic disease management of TCM. Intensive management group were treated with more follow-ups and more education of COPD. The observation time is 3 months. Patients were followed up regularly. Patients’knowledge of disease, attitude, behaviors, medication compliance, CAT and acute exacerbation events were observed every time, nutrition index and pulmonary function was observed before and after the research.Results1、60 cases were recruited. One patient fell off in general group.59 cases were taken into analysis, which 30 cases in intensive group,29 cases in general group.2、The score of the knowledge part of COPD scale had statistical difference within two groups in multi follow-up times (P<0.05), but the attitude and healthy behavior part have no statistical difference (P>0.05). The score of knowledge part also had statistical difference between two groups (P<0.05), but no significant difference was shown in attitude and healthy behavior part (P>0.05). However, comparing before and after the research the score of all parts of the COPD scale were statistically different in the intensive group (P<0.05)3、The score of medication compliance scale had statistical difference within two groups in multi follow-up times (P<0.05). It also had statistical difference between two groups (P<0.05)4、The score of CAT had statistical difference within two groups in multi follow-up times (P<0.05).But it had no statistical difference between two groups (P>0.05).5、After the chronic disease management of TCM there is no significant difference in BMI index, hemoglobin, albumin and FEV1 between two groups (P>0.05)6、After the chronic disease management of TCM patients in both groups cut down smoking (P<0.05). There is no difference between two groups (P>0.05).7、The satisfaction of chronic disease management of TCM is significantly different between two groups (P<0.05)ConclusionIncreasing the follow-up times of chronic disease management of TCM can improve the patients’life attitude and life style, patients’knowledge of COPD, their compliance and clinical symptoms. However, the general group also has an effect on improving patients’compliance and clinical symptoms. Doctors and nurses are all busy at work. In the shortage of staff, adding contents of the management, for example learning the knowledge of COPD, regularly taking drug and self management education can achieve the effect of intensive management and reduce clinicians’ daily work.
