The paper is composed of literature review and clinical research.Literature reviewLiterature review of western medicine systematically summarized the studies of chronic obstructive pulmonary disease (COPD) integrated management in definition, goal, content and evaluation index. Literature review of Traditional Chinese Medicine summarized the integrated management of chronic diseases in theoretic origin and content. The review also introduced the application of Traditional Chinese medicine integrated management in diabetes, hypertension, and COPD.Clinical researchObjective:To evaluate the clinical efficacy of Cycle Management in patient with stable COPD.Methods:Stable COPD patients that fit the diagnosis standard of the case, were randomly divided into management group and control group. The study population consisted of 180 patients, but 15 cases were lost. Finally, there were 165 cases that have complete results and statistical significance left. The Management group has 85 cases while the control group has 80 cases when the research was over. The control group was given treatments based on the standard treatment of "Guide of COPD (2007)". Patients in the Management group were established COPD electronic files and were treated with health education in traditional Chinese medicine, Jian Shen Qi Gong Ba Duan Jin or regular exercise and medication manage. COPD Assessment Test (CAT), the modified Medical Research Council (mMRC) Dyspnea Scale, number of acute exacerbation and lung function were compared between the two groups.Results:Compared to control group, there was significant improvement in CAT score, mMRC Dyspnea Scale and number of acute exacerbation(P<0.05) in management group. CAT score of management group decreased from (18.54±6.955) to (13.53±6.996). Postbronchodilator forced expiratory volume in one second (FEV1) to forced vital capacity (FVC) ratio was improved in management group compared to the baseline. Compared Jian Shen Qi Gong Ba Duan Jin and general exercise subgroup of management group based on the exercise plan, Jian Shen Qi Gong Ba Duan Jin had significant effect on CAT score (P<0.05). Compared inhalation and non-inhalation subgroup of management group based on inhalation treatment, inhalation had advantage on improvement of FEV1 as percent predicted for age, gender and height (FEV1% predicted) (P<0.05). Inhalation subgroup of management group showed better efficacy in CAT score and number of acute exacerbation than control group (P<0.05).Conclusion:Cycle management alleviated the symptoms of patients with stable COPD, improved the quality of life, and reduced the risk of exacerbation. Cycle management is more comprehensive than single drug management.
