Basic Evaluation Of TCM In The Use Of Chronic Disease Management For COPD Patients Who Complicate With Pulmonary Heart Disease

ObjectivesChronic obstructive pulmonary disease(COPD) is one of the main chronic diseases that are harmful to human health. It always complicate with pulmonary heart disease, has the characteristics of high disability rate and high mortality rate. COPD has standardized treatment plan, but traditional medical services alone is difficult to stabilize the the patient’s condition.The establishment of disease management is the trend of the development of global medical. Traditional Chinese medicine(TCM) has great potential in the prevention and treatment of pulmonary disease.Explore the practical application of TCM in the chronic disease management of COPD patients combined with chronic pulmonary heart disease, and the influence on the clinical symptom, quality of life, laying the groundwork for the further optimization of chronic disease management program.MethodsCollect the data of the included COPD combined with pulmonary heart disease patients. Early intervention and health education training for patients were done by interviews, distributing instruction manuals and telephone contact(including treating winter disease in summer, the Chinese Medicined Diet, eight pieces of brocade or breathing exercise).The patients were observed for 9 months. Regular visits to remind patients with related training guide, and tracked patient compliance(once per month). Taking the input data for the baseline questionnaire, and use some.indicators to evaluate the health condition, such as:COPD combined with pulmonary heart disease in TCM PRO scale, COPD assessment test, modified British Medical Research Council, adult COPD quality of life scale(QOL) etc. Analyse the change of all the patients’indicators, record the times of acute exacerbation of all patients throughout the study condition, evaluate the feasibility and clinical efficacy of the integrated traditional Chinese medicine intervention in the management of chronic disease, guide the clinical chronic diseases management.ResultsThe CAT, PRO, QOL scores got from the patients who were observed by the program for 9 months show that the average PRO score decreased by 0.75, the average CAT score decreased 0.45, the QOL score decreased by an average of 0.09.The QOL indicators difference were consistent with the normal distribution, the other indicators difference were not consistent. The two related sample Paired t test and the two related sample Wilcoxon signed rank test both show that P values were less than 0.05, the results were statistically significant. It means the TCM Comprehensive intervention can significantly improve the patients’symptoms and quality of life.ConclusionThe study shows that TCM can significantly improve clinical symptoms and quality of life of patients who had COPD with pulmonary heart disease.
