Chronic Disease Management Of Traditional Chinese Medicine On The Impact Of COPD Patient’s Pulmonary Rehabilitation Compliance And The Clinical Curative Effect

ObjectivesThe therapy of COPD, as we know, is not only limited to the acute exacerbation but also the stabilization period. With the treatment of pulmonary rehabilitation services by TCM, many COPD patients can not only stable and reverse the pathophysiological changes which in turn prevent th function of lung from deterioration, but also improve the quality of their life. Yet it has been seen that the effects of the pulmonary rehabilitation services are far from achievement we expected. With comprehensive chronic disease management of traditional Chinese medicine, we can improve the compliance of COPD patients’pulmonary rehabilitation. Therefore, based on the randomized controlled trial, our study analyses the relationship between Pulmonary rehabilitation of TCM and efficacy of COPD; Chronic Disease Management and efficacy of and the compliance of COPD patients’pulmonary rehabilitation and the clinical curative effect, which can provide some basis for the management of chronic diseases system.MethodsThe data was based on collecting the medical records from ward of a hospital and outpatient clinic COPD patients who are in the stable phase. As for the study, it took corresponding to diagnosis standards and randomly assigned them to different conditions. Patients in the treatment group were given conventional treatments and intervention measures of integrated TCM management whereas patients in the control group were given conventional treatments only. Symptom score of COPD patient was taken notes by daily diary card and evaluate the efficacy of rehabilitation at different time points before and after treatment for a three-month. Results1. General situation analysis:60clinical cases in line with the inclusion criteria. Patients in the treatment group at the age of45-84age distribution, and Patients in the control group at the age of39-81age distribution.2.The execution of pulmonary rehabilitation analysis:the execution of pulmonary rehabilitation was significantly different between2groups throughout the study (P<0.05) which indicating that traditional chinese medicine management of chronic diseases can improve the compliance of the COPD patient.3. Symptom score of COPD patient analysis:After treatment, patients in treatment group with a higher score than the control group, and there is a statistically significant difference. We can inference on the result is that traditional chinese medicine management of chronic diseases can improve patient’s pulmonary rehabilitation.4. Relationship between ages and compliance of pulmonary rehabilitation analysis:The results suggest that ages have nothing to do with pulmonary rehabilitation, and there was not a statistically significant difference between them (P>0.05) which indicating that COPD patients of all ages have the same compliance.5. Relationship between education levels and scores of compliance of pulmonary rehabilitation analysis:We divided them into two group, one is less than secondary education, and the other one is patient with or more than secondary education. Patients in treatment group with a higher score than the control group, and there was a significantly different between2groups (P <0.05) which indicating that Chronic Disease Management of TCM can improve patient’s treatment adherence which was independent of educational background.6. Within class analyze of the CAT scores before and after treatment:After treatment, patients in treatment group with CAT score are decline and the difference was statistically significant (P<0.05) which tell us that pulmonary rehabilitation in TCM management do good to COPD patient. Although CAT score is decline among the patients in the control group, there was no statistically significant difference (P>0.05), which indicated that patients in the control group, pulmonary rehabilitation can relieve their discomfort, but efficiency is lower. 7. Factors of efficacy and prognostic evaluation analysis:based on Principal component analysis, we can conclude that ages, percent of breathing completion, percent of the chest massage completion, percent of exercise called Ba Duan Jin completion, total scores of pulmonary rehabilitation and CAT scores after treatment were not the sole independent predictor. We can conclude that there are The First Factor contains percent of breathing completion, percent of the chest massage completion, percent of exercise called Ba Duan Jin completion and total scores of pulmonary rehabilitation, which can reflect the curative effect of pulmonary rehabilitation; The Second Factor contains ages and CAT scores after treatment which can reflect patients’conditions.GonclusionThe study, through methods of observation, analyses the execution of pulmonary rehabilitation between two groups. Above results suggest that Pulmonary rehabilitation can attenuated symptom of COPD which had nothing to do with age; Chronic Disease Management of TCM can improve patient’s treatment adherence which was independent of educational background, and there were two factors accounting for efficacy of pulmonary rehabilitation. The first factor, reflecting the main curative effect evaluation, was54.81%of the whole. And the second factor, reflecting the patients’conditions, was26.51%of the whole. To some certain extent, our studies provide some basis for the management of chronic diseases system and used to guide clinical practice.
