ObjectiveUnderstanding the status and influencing factors of core competence in critical disease nursing of cancer hospital’s low Seniority nurses.This study is based on tumor specialized characteristic and the low qualification nurses’ core competence, to build core competence in critical disease nursing of cancer hospital’s low qualification nurses by quantitative research and qualitative research, providing a reference for on-the-job training of core competence in critical disease nursing of cancer hospital’s low qualification nurses.MethodsThis study adopts the convenient sampling method on the basis of consulting a large number of literature, and selected nurses of two tumor hospital in Henan province. Understanding the status and influencing factors of core competence in critical disease nursing of cancer hospital’s low qualification nurses by questionnaire survey and statistical analysis after the questionnaire survey. Semi-structured interview adopts purposeful sampling method,7 nursing managers of a tumor hospital are selected as the research object to do depth interview, then analysis the interview data. According to the results of the questionnaire and semi-structured interview, expert questionnaire are accomplished, then two rounds of expert consultation are developed, including 20 senior experts in the field of the nursing management, clinical nursing, nursing education, critical medical, preliminary building a training outline of core competence in critical disease nursing of cancer hospital’s low qualification nurses, which include 5 training programs and 25 training content.Results1 The total score of the core competence in critical disease nursing of cancer hospital’s low qualification nurse is (129.49±34.57), and sorting from high to low of the dimensions of core ability is:communication and coordination ability, critical thinking ability, management ability, professional practice ability and professional development ability.2 The single data analysis of the core competence in critical disease nursing of cancer hospital’s low qualification nurse showed that:work hospital, working years, the first record of formal schooling, the opportunity of nursing critical patients and core ability of the department was statistically significant (P< 0.05).3 View the core competence in critical disease nursing of cancer hospital’s low qualification nurse as the dependent variable, and 10 related factors as independent variables, the variables Inclusion criteria a=0.05, variable exclusion criteria a=0.10, multiple linear step wise regression showed that Partial regression coefficient of "working years is 15.488 (P<0.05).Partial regression coefficient of job unit is-25.784 (P<0.05), Based on ICU, Partial regression coefficient of Department of breast is-34.713 (P<0.05), Partial regression coefficient of Department of radiotherapy is-21.936 (P<0.05).4 Seven core competence in critical disease nursing of cancer hospital’s low qualification nurse are refined through the seven experts interview:professional attitude, critical theory and critical practice skills, critical thinking, management ability, communication and coordination ability and professional development ability.5 During the two rounds of core competence in critical disease nursing of cancer hospital’s low qualification nurse expert consultation, the positive coefficients is 100% and 90%; the experts’authority coefficient average was 0.86±0.08,; After two rounds of expert consultation, the coordination coefficient of primary index of 0.370, the coordination coefficient of the secondary index 0.382, chi-square test result is P< 0.05. The authority of experts and enthusiasm is well, consulting results are in good coordination degree and reliable results.6 The training outline of are constructed after two rounds of expert including training target, training and implementation plan, five training programs,25 training content,training target, training mode, evaluation method and the training hours.Conclusions1 The level of score competence in critical disease nursing of cancer hospital’s low qualification nurse is relatively low, and the lowest is the professional practice and professional development ability, which need to be further improved,2 The main factors of score competence in critical disease nursing of cancer hospital’s low qualification nurse is years of working、the work unit、Work Department.3 Establishing the training outline of core competence in critical disease nursing of cancer hospital’s low Seniority nurse, including training target, implementation plan, fivetraining programs,25 training content and training target, training mode, evaluation method and the training hours. Providing a reference for on-the-job training of core competence in critical disease nursing of cancer hospital’s low qualification nurses.
