ObjectivesTo understand current situation and influencing factors of nursing core competency of critical diseases care among low seniority nurses.To evaluate the effect of systematic training on improving nursing core competency of critical diseases care among low seniority nurses and develop the training model.MethodsIt is a test-retest and quasi-experimental research design. Eighty low seniority nurses working in Xiangya hospital were recruited,who participated in the systematic training of critical diseases related knowledges among low seniority nurses during2011-2012. All of them received the systematic training on common clinical critical diseases related knowledges and skills for14months. The training program had two sections. The first section was the systematic training of the clinical critical diseases related theories during the first seven months with a total of51hours.The second section was the comprehensive simulative training of the clinical first aid skills for the following seven months. This study was just to evaluate the first section training effect. It was divided into there stages.The first stage:a baseline survey.Before the intervention, using the general information form collect general informations. At the same time,an evaluation of nursing core competency of critical diseases care among low seniority nurses,general self-efficacy and an examination were conducted in80nurses.The secend stage:Implementation of training program.The intervention was systematic training of critical diseases related knowledges among low seniority nurses witch lasted for seven months.The training contents included requirements of core competency among low seniority nurses and the common clinical critical diseases related knowledges.The training methods included lectures and discussion groups.The third stage: Evaluation of effectiveness after training.At the end of intervention, a survey about nursing core competency of critical diseases care among low seniority nurses and an examination were conducted. To evaluate the effect of the intervention. All the80nurses completed the questionnaires under the unified guidance.The questionnaires were designed on the basis of consulting a large number of literature.And it has been revised through expert evaluation and pilot study.The questionnaires included general information form, questionnaire of nursing core competency of critical diseases care among low seniority nurses,general self-efficacy scale.SPSS18.0was adopted for data entry and statistical analysis. Statistical methods included descriptive statistics, ttest, Ftest, Pearson correlation coefficients, multiple linear regression analysis.Results1.Before training, the average score of the test about critical diseases related knowledges among low seniority nurses was49.93±5.87.The average score of nursing core competency of critical diseases care among low seniority nurses was136.08±28.29.The mean scores of the dimensions of nursing core competency of critical diseases care among low seniority nurses arranged in descending order as follows: communication and coordination ability;critical thinking ability; management ability;professional practice ability;professional develop-ment ability.2.Before training, the average score of general self-efficacy among low seniority nurses was28.77±4.652. 3.One-way analysis of variance showed that the average scores of nursing core competency of critical diseases care among low seniority nurses had significant differences (P<0.05) among different education background, professional title, opportunities to care critical patients in work,training experiences,clinical working time and working departm-ent.4.There was significant correlation between nursing core competency of critical diseases care and general self-efficacy among low seniority nurses (r=0.494,P=0.000)5.Multiple linear regression analysis showed that the general self-efficacy, the education background, the opportunities to care critical patients in work were the main factors (P<0.05).6.Before training, the average score of the test about critical diseases related knowledges among low seniority nurses was49.93±5.87. After training, it was increased to63.93±6.30.The difference was statistically significant (P=0.000).Before training, the average score of nursing core competency of critical diseases care among low seniority nurses was136.08±28.29.After training, it was increased to141.36±26.88.The difference was statistically significant (P<0.05)Conclusions1.Nursing core competency of critical diseases care among low seniority nurses should be further improved. Professional development ability was the lowest.2.The general self-efficacy among low seniority nurses was at upper level.3.The main impact factors of nursing core competency of critical diseases care among low seniority nurses were the general self-efficacy, the education background and the opportunities to care critical patients in work.4.Systematic training can improve nursing core competency of critical diseases care among low seniority nurses.
