ObjectiveGastric cancer is the digestive tract tumor is the most common malignant tumor,accounting for the digestive tract, though the treatment of gastric cancer have greaterprogress, but the prognosis of patients with gastric cancer, improve is still not clear,especially the stomach cancer patient died of tumor recurrence. Gastric cancer recurrencebecome influence the prognosis of patients with cancer of the important factors. The studyevaluates the prognostic influence of perioperative blood transfusion on gastric cancerpatient to provide experimental reference for clinical surgeon.MethodsWe selected248patients with gastric cancer who underwent resection with curativeintent at the Rongcheng Renmin Hospital and Rongcheng Traditional Medicine Hospitalfrom1January2004to1January2007.248patients who underwent curative gastrectomyfor gastric cancer were retrospectively reviewed and clustered into three groups:with bloodtransfusion (>4u) group A,with blood transfusion (<4u)group B or without bloodtransfusion group C. Statistical analysis of the correlation of blood transfusion andclinicopathological features and the prognostic influence of perioperative blood transfusionon gastric cancer patient were performed.Results248patients were included in this study.171men (71.3%) and71women (28.7%) wereincluded,with a median age of66(range27-89) years at the date of surgery. There were nosignificant differences between the three groups in the aspect of patient, tumour andtreatment characteristics. The1,3,5-year disease recurrence rates were14.63%,34.02%and51.22%in the group A, and14.71%,30.88%and39.71%in the group B, and11.22%,28.57%and35.71%in the group C respectively. Survival analysis showed that thisdifference was highly significant. In the univariate analysis, blood transfusion were significantly related to disease recurrence.No significant relationship was found between theamount of transfused blood and prognosis.Subgroup analyses of prognosis according tostage showed significant difference in stages II and III between group A and groupC.Subgroup analyses of prognosis according to anemia showed significant difference ingroup A and group C. Subgroup analyses of prognosis according to with or withoutchemotherapy showed significant no difference in group A and group C.In multivariateanalysis, blood transfusion remained an independent prognostic variable for gastric cancerrecurrence as were TNM stage and chemotherapy.ConclusionPerioperative blood transfusion is associated with a significantly worse prognosisfollowing gastric cancer surgery.
