Objective:Allogeneic blood transfusion(ABT)is sometimes unavoidable in radical gastrectomy for gastric cancer. The purpose of this retrospective analysis was to investigate the association between perioperative blood transfusion and postoperative infection of gastric cancer, focusing on the effects of blood transfusion, blood transfusion and white blood cell content on postoperative outcomes, and providing a reference for reducing postoperative infection and other complications. Methods:The clinical and pathological data of 964 patients with radical gastrectomy from January 2009 to December 2018 in the Department of General Surgery,Second Affiliated Hospital of Shandong First Medical University were collected.Analyze the general condition of the patient’s gender, age and body mass index;collect perioperative blood transfusion and postoperative infection, and use SPSS 22.0 for statistical analysis between perioperative and postoperative infections. The association, and subgroup analysis of blood transfusion time and blood transfusion volume and white blood cell content, provide a basis for effective avoidance and reduce the incidence of postoperative infection complications. Results:Of the 964 patients,392(40.6%)received transfusion therapy during the perioperative period. After one-to-one matching,361 patients were further analyzed, and 68(9.4%)of them developed postoperative infection.Subgroup analysis demonstrated intraoperative blood transfusion, high volume(>7.5 U),and no leukocyte removal, which is a risk factor for postoperative infection in gastric cancer.Conclusion:1.This retrospective analysis identified propensity transfusion as a risk factor for postoperative infection in gastric cancer by propensity score matching and subgroup analysis.2.Intraoperative blood transfusion, high volume(> 7.5 U),no leukocytes,is a high risk factor for postoperative infection in gastric cancer. In order to reduce infection,it should be avoided as much as possible to give a large volume of blood transfusion without removing leukocytes during surgery.
