Objective:To investigate the impact of perioperative allogeneic red blood cells transfusion(RBCT)on the early recurrence and early survival of patients with hepatocellular carcinoma(HCC)after liver resection.Method: A total of 999 patients who underwent potential curative resection of HCC from July 2013 to July 2016 were retrospectively enrolled.Patients were divided into the transfusion group and non-transfusion group according to whether or not they have allogeneic RBCT during perioperative period.Propensity score matching(PSM)was used to minimize the selection bias due to unbalanced baseline characteristics between the two groups.Kaplan-Meier was used to compare the differences of early recurrence and early survival.Univariate and multivariate Cox regression analyses were used to screen the independent risk factors for early recurrence of HCC patients.Result: There were 100 patients in red cell transfusion group and 899 patients in non-red cell transfusion group.After PSM,85 pairs of patients were successfully matched,and there was no significant difference in baseline data between groups.Before PSM,the early recurrence rate and early survival rate of the red blood cell group was significantly higher than that of the non-red blood cell group(P<0.05).However,there was no significant difference in early recurrence and early survival rates between the two groups after PSM(P>0.05).Multivariate analysis showed that perioperative allogeneic red blood cells transfusion was not an independent risk factor of early recurrence for patients with HCC after liver recurrence(P=0.153).Conclusion:Perioperative allogeneic RBCT may has no impact on early recurrence and early survival after hepatectomy for HCC,and it is not an independent risk factor for early recurrence for patients with HCC after liver recurrence.
