BackgroundAcute myocardial infarction(AMI) with high incidence and high mortality rates has become the number one killer to human health.So accurate, timely, and effective diagnosis and treatment of AMI is the key to reduce mortality, reduce complications and improve the prognosis of patients, being the world’s research focus.Myeloperoxidase (MPO)has strong oxidative stress and promote the inflammatory response.Increased expression and activity can promote the formation of atherosclerosis, vascular endothelial cells, extracellular matrix degradation, lipid oxidation protein, affecting the stability of plaque and so on. Myeloperoxidase plays an important role in AMI’s occurrence and development.ObjectiveUsing the immunohistochemical detection the concentration of MPO in rats with AMI and clinical AMI patients within two hours of onset at different time points,observed whether MPO existed concentration time curve in the diagnosis of AMI and MPO expression in AMI rat myocardium.To investigate MPO in early diagnosis of AMI patients risk stratification and clinical significance in prognosis.Methods1.Animal experiments:The150SD rats were randomly divided into surgical group (90) and control group (60).Ligatured surgical group rats’left anterior descending coronary artery to be AMI model.Control rats only in the corresponding parts of threading, but without ligation.Respectively, after2hours,4hours,6hours,10hours,16hours,24hours,48hours,72hours to extract the right ventricle into the blood5ml heparin tube, centrifuged to remove the supernatant at-70degrees refrigerator waiting along with Elsai technology to detedt MPO plasma concentrations.Rats were sacrificed.Then removed their hearts, fixed in10%formalin solution, embedded in paraffin, stained with HE and analyzed with immunohistochemical.2.clinical research:Selected within two hours of onset of AMI patients20.11patients in normal control group, respectively, in two hours,4hours,6hours,10hours,16hours,24hours,48hours,72hours after the onset took5ml venous blood into heparin tube.Then measured MPO plasma concentrations of acute myocardial infarction group and normal control group by ELSAI.Both groups were also collected age, sex, smoking history, alcohol consumption, hypertension, diabetes mellitus, the same testing leukocytes, neutrophils, total cholesterol, low-density lipoprotein, high density lipoprotein, creatine kinase isoforms enzymes, troponin I and other indicators, using Pearson correlation analysis described above MPO index correlation.3.Statistical analysis:Used SPSS17.0statistical software operating systems, measurement data with the mean±standard deviation (±S) said.To compare the two groups used independent samples t test.Count data comparison used x2test.P<0.05was considered statistically significance.MPO with age, white blood cells, neutrophils, high density lipoprotein, low density lipoprotein correlation used Pearson correlation analysis. MPO plasma concentration time curve used linear graph.Results1.Animal experiments:①Yellow particle deposition could be seen in AMI rat cardiac cells cytoplasm and nucleus.MPO expression was positive, while the myocardial tissue of rats in the control group no yellow particle deposition.②.Two groups of rats after surgery2hours to72hours comparison of mean plasma MPO in each blood collection point showed without statistical significance promise (P>0.05),③.There was no obvious peak plasma concentrations of MPO in rats both in the experimental group and the control group after surgery2-72hours.And concentration time curve was relatively flat.2.Clinical research:①AMI patients with venous plasma MPO levels were significantly increased.There was a significant difference (P<0.05) compared with the control group.②AMI after the onset of2-72hours both appeared to increase plasma concentrations of MPO.However,there was no significant concentration peak and the concentration time curve flat.③. MPO activity in plasma of patients with AMI had correlation with neutrophils (r=0.540, P<0.05), low-density lipoprotein (r=0.442, P <0.05), total cholesterol (r=0.384, P<0.05), creatine kinase0=0.621, P<0.05), and there was no significant correlation with age, sex, high-density lipoprotein, troponin, etc.Conclusions1. Yellow particle deposition had been seen in AMI rat model of Myocardial cell cytoplasm and nucleus section which standed for MPO.MPO’s expression was positive.2.MP0significantly increased in the early AMI (within2hours).Within72hours after the onset of myocardial infarction MPO had been maintained at a high level, without significant peak and concentration time curve relatively flat. The plasma concentration of MPO and neutrophils, low-density lipoprotein, total cholesterol, creatine kinase.3.MPO has important significance in the early diagnosis of acute myocardial infarction.But it is not right to determine the time of myocardial infarction in accordance with the MPO concentration.
