Objective:By observation of the clinical effect of functional exercise of patients with knee osteoarthritis underwent needle knife surgery, explore the significance of functional exercise in patients with knee osteoarthritis in the rehabilitative treatment is effective and functional exercise of knee joint bone maintenance on the curative effect of acupotomy therapy, to provide the clinical basis for the Acupotomy combined with functional exercise therapy osteoarthritis of the knee.Methods:Following the principle of randomized controlled, according to inclusion, exclusion criteria to select the Third Affiliated Hospital of Beijing University of Chinese Medicine from2013January to2014February in Department of orthopedics, outpatient and inpatient admitted to meet the diagnostic criteria of knee osteoarthritis patients with50cases, were randomly divided into training group and control group with25cases in each. Functional exercise group25cases,11were male,14female; age42-76year old, average55.95±7.36years;16cases of knee,9knees; the course of disease was4months-16years, mean7.32±3.54years; body mass index was23.72±2.69. A control group of25cases,9were male,16female; age44-79year old, average61.30±6.52years;15cases of knee,10knees; the course of disease was1years-21years, mean9.41±4.26years; body mass index was24.31±3.24. There was no significant difference in gender, age, course of disease, body mass index, VAS score, patellar movement, Lysholm knee score in two groups of patients before treatment (P>0.05), comparable. Control group: the needle knife therapy alone (treatment2-4, treatment time,1-2per week for4weeks.) Functional exercise group:the needle knife treatment started second days after combined with functional exercise (3times, every day as early as in exercise4weeks). Before treatment,4weeks after the treatment (the end of a course of treatment),8weeks after the treatment (follow-up) by visual analogue scale (visual analogue score, VAS), patella mobility and Lysholm knee score (Lysholm knee score standard, LKSS) the clinical curative effect of three assessments, recording observation data, application of SPSS17.0software for statistical analysis.Results:1.VAS score within each group (two groups were basically the same)4weeks after treatment compared with before treatment, P<0.01, significantly, there was statistical significance;8weeks after treatment compared with before treatment, P<0.01, significant difference was statistically significant; after the treatment,8weeks and4weeks after treatment comparison, P<0.05, the difference has statistical significance. Comparison group:4weeks after treatment between the two groups, P>0.05, the difference was not statistically significant, there was no significant difference in improving the knee joint function of daily life after a course of two methods;8weeks after treatment between the two groups, P<0.01, significantly, with statistical significance, functional exercise group than in control curative effect is more stable, more durable, Acupotomology postoperative functional exercise with no effect cannot consolidate, maintain.Conclusion:1Needle-knife therapy alone and acupotomy surgery with two methods of functional exercise can relieve pain, improve the mobility of KOA in patients with patellar, improve knee function of daily life. The end of2after one course of treatment, two methods to relieve pain, improve knee function of daily life:there are no significant differences in the improvement of the patella; but the movement degree plus functional exercise group was significantly better than the acupotomy group. The end of3after one course of treatment, after4weeks, plus functional exercise group significantly better than the simple acupuncture therapy group, especially in improving knee daily life function, improve the degree of patellar movement. The exercise group than in the control group curative effect is more stable, more durable, Acupotomology postoperative functional exercise with no effect cannot consolidate, maintain. Functional exercise to make up for the needle knife therapy to relieve pain quickly, instant effect is good, but the long-term effect is insufficient, so that lasting effect. Acupotomy combined with functional training for KOA short-term, long-term efficacy, safety, high stability, it is worth in clinical application.
