Investigation On Community-Based Rehabilitation Utilization Status And Influencing Factors Of The Physical Disabled Persons In Changchun

Objectives:The purpose of this study is to understand the current status of unltilization ofcommunity-based rehabilitation services for the physically disabled persons inChangchun; and analysize the factors affected its utilization, which will provid a basisfor the future improvement of community-based rehabilitation work.Methods:305physically disabled people from12community health service centers ofChangchun met the inclusion criteria were recruited in this study. A self-designedquestionnaire was used to investigate the utilization of each community-basedrehabilitation service. The questionnaire is consist of two parts, including: generalinformation of participants and the utilization of community-based rehabilitationservice (basic services of communities, treatment and training services, education andvocational services and social function rehabilitation services). The database wasestablished by EpiData3.02, and the data was statistically analyzed by using SPSS17.0Descriptive analysis was conducted on the disabled persons’ general information andutilization status of community-based rehabilitation services. Chi-square test was usedfor the factors affected the utilization by single factor analysis method, and logisticregression was used for multiple-factor analysis.Results:1. The utilization rates of basic rehabilitation services, treatments and trainingservices, education and vocational services, social function rehabilitation servicesamong physically disabled persons in Changchun was41.3%,32.3%,21.7%and26.7%respectively in2013. 2. The reasons for the inadequate usage of the basic services were: physicaldisabilities led action inconvenience (44.9%) and without knowing this service(43.8%). The reasons for the inadequate usage of treatment and training services were:did not know this kind of service (71.4%) and could not afford the fees (38.4%). Thereasons for the inadequate usage of education and vocational services were: thephysically disabled persons doubted about the therapeutic effects of this kind ofservice (80.9%) and could not afford the fees (58.3%). The reasons for the inadequateusage of social function rehabilitation services were: the disabled persons were afraidof being discriminated (39.1%) and did not know the community had this kind ofactivities.3. The result of Chi-square test showed that: There was a significant differencebetween gender and the utilization of social function rehabilitation services amongphysically disabled persons (P<0.05). Different ages has a significant difference in theutilization of basic services, treatments and training services, education and vocationalservices among physically disabled persons(P<0.05). Different level of education hasa significant difference in the utilization of basic services, education and vocationalservices among physically disabled persons(P<0.05).Different marital status,anddisabling reason has a significant difference in the utilization of education andvocational services among physically disabled persons(P<0.05).Different familymonthly income, and disability grade has a significant difference in the utilization ofbasic services, treatments and training services and social function rehabilitationservices among physically disabled persons(P<0.05).Different disabling ages, degreeof independent living, working state, and whether has an insurance or not has asignificant difference in the utilization of basic services, treatment and trainingservices, education and vocational services and social function rehabilitation servicesamong physically disabled persons(P<0.05).4. The result of logistic regression analysis showed that: disability grade,disabling ages, working status and whether has an insurance or not are added in thebasic services using regression model and its prediction accuracy is82.3%;familymonthly income, disabling ages and working status are added in the treatment and training services using regression model, and its prediction accuracy is84.3%;ages,family monthly income and disabling reason are added in the education andvocational services using regression model, and its prediction accuracy is82.7%;family monthly income, disabling ages, level of independent living, working statusand gender are added in the social function rehabilitation using regression model, andits prediction accuracy is76.7%.Conclusions:1. The utilization of the services for the community-based rehabilitation is low inChangchun, and the utilization of the education and vocational services is the lowest.It is important to further improve the utilization rate of community services,especially the use of education and vocational rehabilitation services.2. The reason for the inadequate utilization in all services is: economic factors,the physically disabled persons did not know that community has rehabilitationservice and did not believe the function of the community-based rehabilitation, thephysical disabilities action inconvenience and were afraid to be discriminated.3. Family monthly income, disabling ages, working status and whether has aninsurance or not were the key influencing factors of the utilization of thecommunity-based rehabilitation services.4. It is necessary to develop more targeted and practical rehabilitation plans formeeting the different characteristics of the physical disable people’ needs and makemore people understand the significance and full use of it for the benefit of morephysical disable people.
