Objective:To study on the relationship between the main distribution of Traditional Chinese Medicine (TCM) syndrome types and correlative factors in patients with diarrhea-irritable bowel syndrome (IBS-D) in Xiamen area. The epidemiological characteristics, clinical data of TCM syndrome types in IBS-D were explored to improve the therapeutic effects.Method:Using the clinical epidemiological method,298cases with IBS-D who first seeked medical advice were selected as the research subjects to investigate the TCM syndrome types, specific symptoms, signs and related factors of them. All the data were analyzed by statistics.Results:1. In the observation cases, there were96cases of liver depression and spleen deficiency syndrome,69cases of yang deficiency of spleen and kidney syndrome,60cases of spleen stomach damp heat syndrome,41cases of syndrome of combined with cold and heat,27cases of weakness of the spleen and stomach syndrome. There was a significant difference (P<0.05) between syndrome types. Syndrome types of liver depression and spleen deficiency were significantly more than other syndromes (P<0.05).2. There were no gender differences between TCM syndromes types(138males and155females, P>0.05). There were age differences between TCM syndromes types (P<0.05), age of onset concentrated in the30-49years of age (P<0.05).3. The main symptom of each syndrome types are as follows:(1) syndrome of stagnation of liver qi and spleen deficiency:abdominal distension and pain, bowel, diarrhea, belching.(2) yang deficiency of spleen and kidney, abdominal distension and pain, form cold, aching lumbus and knees, diarrhea, languid.(3) spleen-stomach damp heat syndrome:abdominal distension and pain, bowel, thirst, diarrhea, Urinating short yellow.(4) syndrome of combined with cold and heat:diarrhea, abdominal distension and pain, thirst, urinate short yellow, form cold.(5) weakness of the spleen and stomach syndrome, inappetence, fatigue, languid, abdominal distension and pain. The main symptoms of each syndrome has a significant difference (P<0.05).4. In terms of lifestyle factors, special hobbies of patients with IBS-D include addicted to spicy food (55.29%), gloomy mood(51.14%), eating disorders(49.83%), cold food(47.20%), and alcohol (46.07%), etc. Work strain and gloomy mood have the highest rates of syndrome of liver depression and spleen deficiency(P<0.05). Strain has the highest rates of yang deficiency of spleen and kidney syndrome and weakness of the spleen and stomach syndrome(P<0.05). Climate change has the highest rates of yang deficiency of spleen and kidney syndrome (P<0.05). Eating spicy and drinking has the highest rates of spleen and kidney syndrome and spleen-stomach damp-heat syndrome (P<0.05).5. Looking into educational background, high school or above which were dominant in number, there were difference in educational background (P<0.05) and obvious differences between occupation (P<0.05). Occupation of onset concentrated was in professionals and the civil services.Conclusion:1. Syndrome of liver depression and spleen deficiency was dominant in IBS-D of TCM syndrome types, it shows that we should pay more attention to soothing liver and strengthening spleen. in the treatment of IBS-D2. Although syndrome of combined with cold and heat did not be added as diagnostic standard by Guidelines for diagnosis and treatment of digestive disease of TCM. But it is one of the main TCM syndromes types of IBS-D. It shows that we should pay more attention to Syndrome of combined with cold and heat, when we treating IBS-D.3. There were no gender differences between TCM syndromes types, the people of young and middle-aged should pay more attention to prevent IBS-D.4. The main symptoms of each syndrome types are as follows:(1) syndrome of stagnation of liver qi and spleen deficiency:abdominal distension and pain, bowel, diarrhea, belching.(2)Yang deficiency of spleen and kidney, abdominal distension and pain, form cold, aching lumbus and knees, diarrhea, languid.(3) Spleen stomach damp heat syndrome: abdominal distension and pain, bowel, thirst, diarrhea, Urinating short yellow;(4) syndrome of combined with cold and heat:diarrhea, abdominal distension and pain, thirst, urinate short yellow, form cold;(5) Weakness of the spleen and stomach syndrome, inappetence, fatigue, languid, abdominal distension and pain.5. In the treatment of patients with IBS-D guidance note life daily life, diet, modern, fatigue and other factors. It is better to play the role of the non-drug treatment and the prevention this disease in TCM. 6. Intellectuals and mental workers should actively prevent from IBS-D.
