Objective:Diabetic nephropathy (DiabetieNephropathy, DN) in diabetes mellitus (diabetesmellitusDM) is one of common microvascular complication and refractory.Once the diabetic patients into the DN clinical stage, will seriously affect their quality of life.This study intends to observe and analysis the correlation of diabetic nephropathy with hyperlipidemia and lipid TCM Constitution type. And then to and provide further theoretical guidance for the prevention and treatment of diabetic nephropathy.Method:In this study,200cases of diabetic nephropathy of stage III and IV hyperlipidemia patients as the object of analysis.The diagnostic criteria for diabetes diagnosis according to WHO1999by DM and diagnosed with type2diabetes mellitus.DN diagnosis was performed according to DN diagnosis of Danish scholar Mogensen staging methods.Diagnostic criteria of blood lipid in reference to the2007May published "Chinese adult dyslipidemia prevention guide".The physique classification using the2005Wang Qi’s "constitution" of the Chinese constitution classification and criteria.The diabetic nephropathy with hyperlipidemia were divided into mild nature, Qi deficiency, Yang deficiency, yin deficiency, phlegm dampness, damp heat, Qi, blood stasis, special intrinsic total nine kinds of constitution.Through the questionnaire survey and blood lipid indexes of physical,Correlation analysis of diabetic nephropathy with hyperlipidemia and lipid in TCM constitution types using statistical method.Result:1.200cases of diabetic nephropathy patients with hyperlipidemia physique type deficiency constitution mainly Yin deficiency and Qi deficiency, solid body mainly for phlegm and blood stasis.2.200patients showed serum total cholesterol (Total Cholesterol, TC) elevated in120cases, triglyceride (Triglyeerides, TG) elevated in144cases, low density lipoprotein (LowdensityliPoProtein eholesterol, LDL-c) elevated in104cases, high density lipoprotein (HighdensityliPoProtein eholesterol> HDL-c) decreased in128cases.3.In the blood lipid indexes, TC between phlegm dampness and peaceful quality showed significant statistical difference (P<0.05) for patients with elevated TC accounts for the largest proportion of phlegm dampness, the lowest proportion of light and matter. LDL-c between damp heat constitution and peaceful quality showed significant statistical difference (P<0.05), LDL-c patients with elevated C accounts for the largest proportion for damp heat, the lowest proportion of light and matter, for there was no significant difference between the index of each type of constitution.4.The body mass index of diabetic nephropathy patients with hyperlipidemia (Body Mass Index, BMI) were observed between each type of constitution, no significant differences in body mass index (P>0.05), but the type of constitution in the phlegm dampness group weight index was the highest, yin deficiency of body mass index minimum.5.For diabetic nephropathy patients with hyperlipidemia waist to hip ratio (Waist hiPratio, WHR) observation, phlegm dampness waist hip is relatively large, flat and the waist to hip ratio between low, phlegm dampness and flat and quality statistical analysis (P<0.05), there was significant difference. In200cases were the patients studied, waist to hip ratio were abnormal in187cases.6.Underweight, normal weight, overweight and obese patients with four HDL-c, apolipoprotein AI (ApolipoproteinA-i, ApoAI) have examined the statistical statistical differences (P<0.05), and other various index change is not obvious.7.200cases of glycosylated hemoglobin (HemoglobinAlc. HbAlc) and biochemical indexes according to the2002Asia Pacific policy type2diabetes group, divided into three groups:that is the ideal group (HbAlc<6.5mmo1/1group (HbAlc), good in6.5,7.5mmo1/1) group (HbA1c>7.5mmo1/1), the three group and TC, TG, HDL-c, LDL-c four index were analyzed and compared. Results TC, TG, LDL-c gradually increased, including TG, LDL-c. HbAIc (>7.5) group increased most significantly; good group and the group and the ideal set TG (P<0.05) with statistically significant difference, good group and the group and the ideal set LDL (P<0.05) with statistically significant difference.8.200cases of patients by age45-60,60-75years of age division. Was found to increase with age, level and quality of damp heat ratio was decreased,vitality deficiency, yin deficiency, Yang Xuzhi along with age increase the proportion increased; patients older than60years with Yin deficiency,vitality deficiency; flat and mass, damp heat in less than60years with the proportion of higher.9.200patients were classified according to sex, of which97were male (48.5%), female103cases (51.5%), diabetic nephropathy of different sex disorder of lipid metabolism in patients with TCM constitution type distribution. The common types of constitution of male patients (greater than20%) for yin deficiency, and common constitution type female patients (more than20%) of vitality deficiency,vitality stagnation constitution of male patients with rare, rare in female patients with damp heat.Conclusion:1. Diabetic nephropathy patients with hyperlipidemia type of constitution, yin deficiency and vitality deficiency mainly to.2. Gender and age effects of diabetic nephropathy patients with hyperlipidemia physical distribution.3. Diabetic nephropathy in patients with hyperlipemia anomalies shown in the reduction of elevated TG and HDL-c.4. In the blood lipid indexes in patients with elevated TC, most should be phlegm dampness; LDL patients with elevated C most should be damp heat, possibility of flat and quality is low, for there was no significant difference between the indexes of each type of constitution.5. HbAIc and TG, LDL-c can ihto a positive correlation. 6. For the overweight patients whose HDL-c, ApoAI levels much lower.
