Objective: With the change of lifestyle and the aging of the society,the prevalence of Type 2 diabetes mellitus is increasing year by year.Diabetic nephropathy is the main pathogeny of chronic kidney disease and renal failure,which not only affects the quality of life of patients,but also brings significant economic burdens to the family and society.The relationship between blood lipid level and diabetic nephropathy in type 2 diabetic patients was discussed by measuring the blood lipid level and the urine microalbumin patients with type 2 diabetes.Methods: 291 patients with type 2 diabetes treated with the endocrinology department of Hebei General Hospital from 6 November 2017 to 30 November 2018 were selected.The general condition of all patients,such as gender,age,height,weight,blood pressure,etc.,was recorded after informed consent.Blood samples were collected from all patients after they had fasted for 8–12 h,blood glucose and blood lipid,liver and kidney function related indicators were measured;urinary microalbumin was estimated.According to the urinary microalbumin results,the patients were divided into three groups,the normoalbuminuria group(NAU group),microalbuminuria group(MIAU group)and macroalbuminuria group(MAAU group).SPSS 22.0 was used to analyze the data.Results:1.Comparison of indicators among the three groupsThere were no significant differences in gender,age,smoking history,FPG,TC,LDL-C,VLDL-C,and ox-LDL among the three groups.Compared with the NAU group,the patients in the MIAU group had longer duration of diabetes,higher SBP and TG levels;patients in the MAAU group had higher levels of Hcy and Lp(a)compared with the NAU group;compared with the MIAU group,in the MAAU group,the duration of diabetes was longer and the SBP was higher;compared with the NAU group and the MIAU group,the patients in the MAAU group had higher BUN and Scr levels,lower eGFR.2.Correlation between clinical indicators and urinary microalbuminUMA was positively correlated with duration of diabetes,SBP,DBP,HbA1 c,TG,Hcy,and Lp(a),but was negatively correlated with HDL-C.There was no significant correlation with age,BMI,FPG,LDL-C,VLDL-C,TC and ox-LDL.3.Logistic regressionUMA was used as the dependent variable(normoalbuminuria group = 0,microalbuminuria group = 1,macroalbuminuria group = 2)to clarify the influencing factors of diabetic nephropathy.Lp(a)is a risk factor for diabetic nephropathy,and HDL-C is a protective factor for diabetic nephropathy.Conclusions:1.The duration,BP and HbA1 c were closely related to UMA in T2 DM patients.2.According to lipid profile,with the increase of TG and Lp(a)level,UMA showed an increasing trend,while with the increase of HDL-C level,UMA showed a decreasing trend.3.Lp(a)was a risk factor for diabetic nephropathy,and HDL-C was a protective factor for diabetic nephropathy.
