Study On The Difference Of TCM Constitution Between Diabetic Nephropathy And Hypertensive Nephropathy

Objective: In this study,we studied the relationship between Constitutions and CKD,and provided a reasonable basis for further Prevention and treatment of progressive renal impairment by TCM constitutions intervention,which was based on the TCM constitutions identification of diabetic nephropathy and hypertensive nephropathy.Methods:This study adopted Kunming Municipal Hospital of TCM`s TCM Constitutions Information processing software(number YNYC2014),for December2015-December 2017 period Kunming Municipal Hospital of TCM Outpatient,cardiovascular endocrine department hospitalization,First affiliated hospital of Kunming Medical University,kidney Department of the DN,HN(chronic renal insufficiency compensatory period)each 150 cases carries on the constitutions of TCM Identification Questionnaire survey,he data carries on the collation,the statistics analysis constitutions of TCM distribution characteristic of DN,the con-stitutions of TCM distribution characteristic of the HN,the constitution of TCM distributing difference of DN and HN two kinds of CKD basal kidney disease.Results: Study Results of Patients with DN 1.1 General Datas:150 cases of DN,101 male,49 female,DN patients average age 58.27+8.08 years.1.2 The constitutions of TCM distribution characteristic of the DN.1.2.1 DN patients with the highest level of transformation scores of the physical(first constitution)Distribution:Qi-deficient Constitution(28%),Phlegm-dampness Constitution(22%),Yin-deficient Constitution(20%),Yang-deficient Constitution (11.33%),Stagnat Blood Contiution(6.67%),damp-heat constitution(4.67%),Stagnat-Qi Constitution(4 %),Balanced Constitution(2.67%),special Constitution(0.67%).1.2.2 The first two biases of DN Patients with 18 kinds of clamping combinations the top 5 in the top two are: Qi-deficient Constitution + Phlegm-dampness Constitution(18.49%),Qi-deficient Constitution+Yin-deficient Constitution(13.01%),Yin–deficient Constitution+ Phlegm-dampness Constitution(11.64%),Phlegm-dampness Constitution + Stagnat Blood Contiution(10.27%),Qi-deficient Constitution + Stagnat Blood Contiution(8.9%).1.2.3 The first three kinds of bias in DN patients had 27 kinds of clamping combinations,among them,the top 3 ranked three kinds of body constitution for Qi-deficient Constitution+Phlegm-dampness Constitution+Yin-deficient Constitution(18.5%),Qi-deficient Constitution+Phlegm-dampness Constitution Phlegm-dampness Constitution+Yang-deficient Constitution(15.08%),Qi-deficient Constitution + Yin-deficient Constitution + Stagnat Blood Contiution(13.01%).2.Study Results of Patients with HN 2.1 General Datas: 145 cases of HN patients,male 94 cases,51 cases of female,DN patients with the average age of 61.42+16.20 years.2.2 The constitutions of TCM distribution characteristic of the HN 2.2.1 HN patients with the highest transformation scores of the constitution(first constitutions)distribution:Yin-deficienct constitution(24.83%),Stagnat Blood Constitution(18.62%),damp heat Constitution(15.86%),Qi-deficienct Constitution(12.42%),Yang-deficienct constitution(6.9%),Phlegm damp constitution(6.9%),Stagnat Qi Constitution(6.21%),Balance Constitution(4.82%),special Constitution(2.06%).2.2.2 The first two biases of the patients with HNthe top 3-digit two kinds of physical fitness:Yin-deficienct constitution + Stagnat Blood Constitution(21.74%),Yin-deficienct constitution + damp-heat constitution(15.22%),Stagnat Blood Constitution + damp-heat constitution(12.31%).2.2.3 The first three kinds of bias in HN patients the top 3 three kinds of physical body are:Yin-deficienct constitution + Stagnat Blood Constitution + damp-heat constitution(20.3%),Qi-deficienct Constitution + Yin-deficienct constitution + Stagnat Blood Constitution(11.6%),Yin-deficienct constitution + damp-heat constitution + Qi-deficienct Constitution(6.52%).3 Comparison of the results of DN and HN patients 3.1 Comparison of the first constitutions in patients with DN and HN:The distribution of Qi-deficienct constitution,Phlegm-dampness constitution,Yin-deficienct constitution,Yang-deficienct constitution,Stagnat Blood Constitution and damp-heat Constitution in DN and HN patients was statistically significant(P<0.05)3.2 Comparison of two types of bias and clip distribution between DN and HN Patients: The distribution differences of Qi-deficienct constitution+ Phlegm-dampness constitution,Qi-deficienctconstitution+yin-deficienct constitution,Phlegm-dampness constitution+Yang-deficienct constitution,yin-deficiency constitution+ Stagnat Blood Constitution,yin-deficiency constitution+damp-heat constitution,Stagnat Blood Constitution and damp-heat constitution were statistically significant(P<0.05).3.3 Comparison of three kinds of bias and clip distribution between DN and HN Patients: the distribution difference of Qi-deficienct constitution+ Phlegm-dampness constitution+yin-deficienct constitution,Qi-deficienct constitution+Phlegm-dampness constitution + Yang-deficienct constitution,yin-deficienct constitution + Stagnat Blood Constitution + damp-heat constitution,damp-heat constitution +Stagnat Blood Constitution + Qi deficienct constitution were statistically significant(P<0.05).Conclusion: 1 The patients with DN and HN have a common body,the conversion score >30,in addition to the first physique for Balance constitution,the basic existence of two kinds of clamping physique and three kinds of clamping physique,because the DN and HN belong to chronic diseases,the disease is complex,only with a single constitution recognition,can not fully reflect the changes in the disease and comprehensive guidance of clinical prevention 2 DN Patients of the first constitution to the highest proportion of Qi-deficienct constitution,two kinds of clamping constitution with Qi-deficienct constitution+ Phlegm and dampness constitution is the highest proportion,three kinds of clamping constitution with Qi-deficienct constitution + Phlegm and dampness constitution + yin-deficienct constitution ratio of the highest;the first constitution of the HN Patients with yin-deficienct constitution of the highest proportion,two kinds of constitution with Qi-deficienct constitution + Stagnat Blood Constitution,three kinds of holding the constitution to yin deficiency + Stagnat Blood Constitution + damp-heat constitution accounted for the highest.It suggests that there are essential differences in the Constitution between DN and HN Patients,and the intervention should be carried out according to the specific physique characteristics.3 The Patients with obesity in DN and HN should intervene according to the characteristics of Qi-deficienct constitution,Phlegm-dampness constitution and damp-heat constitution.4 The urine protein of the patients with phlegm dampness constitution,Qi-deficienct constitution and Yang-deficienct constitution was obvious in the DN and HN,and the clinical urine protein should be treated with Phlegm,Qi deficiency and yang deficiency.
