ObjectiveTo understand the current situation of threatened abortion, pregnancy stressor, social support and coping style in nurses group, and investigate the relationship between pregnancy stressor, social support, coping style and threatened abortion in nurses.Method1. Cross-sectional study: We choosed the nurses who are below the age of35years old with obstetrical history in four hospitals in Bengbu, Anhui Province. Excluding criteria are: nurses who are reluctant to participate in the research; operating room nurses, ICU nurses; suffering from serious chronic diseases such as diabetes, hypertension, and immune system diseases, thyroid disease, genetic disease before pregnancy. The cluster sampling survey study was used here, from June to September2012in the four hospitals. Self questionnaire’pregnant maternal and child health record sheet’was used to collect the social demographic data of pregnant women, including General situation, Habits and customs, Women’s reproductive health, Health history and medical history, Pregnancy history, Pregnancy lifestyle, Pregnancy situation and Childbirth conditions; Source of stress during pregnancy, including Pregnancy stressful life events, Family stress questionnaire, Effort-reward imbalance model scale, Pregnancy-related anxiety; Social support (applied Perceived Social Support Scale); Coping styles (applied Trait Coping Style Scale). The survey investigated a total of349questionnaires.310valid questionnaires are left excluding invalid ones, the efficiency is 88.83%. Socio-demographic variables descriptive statistical analysis; In order to understand the level of pregnancy stressor, social support and coping style in nurses, we use T-test to compare its score with the theoretical values.2. Case-control studySelect threatened abortion nurse (clinically diagnosed) in Cross-sectional survey as case group, excluding criteria:the parents have chromosomal abnormalities, immune disorders, couples blood group incompatibility, genital deformities factors. By1:1matching principle, the control group select cases are:same year childbirth full-term single births live births, no birth defects, birth weight appropriate for gestational age children, the age differ to cases no more than2-years-old, pre-pregnancy female reproductive health same to case. Using χ2test and independent samples t-test analysis, objective to analyze the possible influencing factors of threatened abortion in nurses. Single factor Logistics regression was used to investigate the affection of different pregnancy stressors to threatened abortion, and investigated the interaction affection of different pregnancy stressors on threatened abortion. Using multiple regression, analysis intermediaries or mediation role of social support and coping style between the work pressure, family pressure and threatened abortion. Multivariate Logistic regression model was used to analyze the impact of social support, coping style to threatened abortion.Result1. Cross-sectional study showed:The rate of threatened abortion in nurses was37.70%, it’s higher than the general population incidence. The level of external input, perceived social support and negative coping in nurses are high. The level of return, over investment, family stress, pregnancy-related anxiety, and actively respond in nurses are low.2. Case-control study showed:Weight loss experience, over-investment, family stress, concerned about the self, and pregnancy-related anxiety are the risk factors to occurrence of threatened abortion, OR=2.08(95%CI:1.16~3.71), and2.37(95%CI:1.15~4.86),5.96(95%CI:2.64~13.46),3.26(95%CI:1.69~6.29),2.83(95%CI:1.34~5.97). Further examination of the different stressors interaction affection to threatened abortion shows, compared with low family pressure-low over-investment, high family stress-excessive investment group (OR=3.04,95%CI:1.11~8.34) and high family stress-excessive investment group (OR=15.84,95%CI:4.50~55.79) occurred threatened abortion significantly are higher. Compared with low attention to themselves-low over-investment, high concern to self-high over-invested group (OR=5.93,95%CI:2.11~16.65,) occurred threatened abortion significantly are higher. Compared with Further examine the different stressors on the the threatened abortion interactions found with low family pressure-low over-invest compared to high family pressure-over-invested group (OR=3.04,95%CI:1.11~8.34), high family stress high over-investment in the group (OR=15.84,95%CI:4.50~55.79) was significantly higher in the threatened abortion. Concern themselves with the low-low excessive investment is relatively high attention to themselves-the high over-investment in the group (OR=5.93,95%CI:2.11~16.65,) was significantly higher in the threatened abortion. Concern themselves with the low-low family stress high attention, high concern to themselves-high family stress group (OR=7.50,95%CI:2.87~19.58) occurred threatened abortion is significantly higher in threatened abortion. When examine the intermediaries or mediation affection of social support during pregnancy and coping style, it’s founded that social support and coping style indirectly influenced the occurrence of threatened abortion through work pressure and family pressure. Negative coping and friends support can significantly predict excessive investment, family support and negative coping can significantly predict family pressure. Multivariate Logistic regression analysis showed that pre-pregnancy weight loss experience, family pressure and attention to self were risk factors to threatened abortion.Conclusion The level of nurse’s threatened abortion incidence is at a high level, and nurse’s external input is high, but the return is low, it indicate that the level of working pressure is at a high level. The level of nurse’s negative coping is high, and actively respond is low.and the level of nurse’s family, friends and other support at high, family stress is low. Weight loss, family stress and pregnancy-induced pressure are the risk factor to threatened abortion. There was an interaction among the different stressors. Social support and coping styles can affect the work and family pressure, and then it affects the occurrence of threatened abortion.
