Objective1.To investigate the disease spectrum and the proportion in23569residentpatients from2001to2010in certain tertiary hospital in Anhui Province,so as toconfigure the medical resource reasonably and provide scientific basis and guide for thetreatment and prevention for the diseases.2.To find out influencing factors of hospitalization expenses for pneumonia in order toprovide a reference to rational control of faster increase of medical care fee.Methods1.Statistics analysis was conducted on the database in relation to patients from23569cases selected by stratified sampling throughout2001and2010,and then create anExcel table and input data,So as to investigate the disease spectrum and the proportionin certain tertiary hospital in Anhui Province between2001and2010by retrospectivestudy and statistical analysis.2.The medical record of2808patients with Pneumonia in Certain Tertiary Hospital ofAnhui Province from2001to2010were collected and nonparametric test andmultivariate regression analysis by Dummy Variable were performed to analyze themain influencing factors of hospitalization costs on Pneumonia.Results1.The total number of hospitalized patients was23569among10years,whichraised from1008in2001to3618in2010and showed an increasing trend(P=0.031),thetotal amplification reached258.93%.The cure rate in patients increased steadily with thechange of35.68%in2001to46.24%in2010(χ~2(trend)=128.805,P<0.001),the case fatalityrate showed descending trend,which dropped from1.24%in2001to0.24%in2010(χ~2(trend)=29.153,P<0.001).2.The male-female ratio was1.23:1,which was more than the national average level.Theage range of the majority(57.11%) was more than65years old,there was the eldertendency than ever.3.There was evident difference in the resource of hospitalized patients, the patients with different systemic diseases in seasonal distribution had different characteristics,morepatients were observed in summer than those in the other three seasons.The proportionwent up from27.16%in the upper5years to30.05%in the later5years. Thehospitalized patients on dangerous or urgent conditions suggested the increasingtendency year by year.4.Disease of respiratory system,injures,pregnancy, childbirth and the puerperium werethe major systematic diseases,disease of respiratory system occupied the sizableproportion ever since the decade.The patients with injures indicated a rising trend.Themonopathy composition had a notable feature ever since the decade,patients withpneumonia,trauma,obstructed labored and acute appendicitis always occupied theleading position of monopathy.The difference of diseases proportion between thehospital and the city hospitals and county-level hospitals both were significant(P<0.05).5.Hospitalization expenses of patient with Pneumonia raised from3515.11in2001to5555.02in2010,which showed an increasing trend year by year, among the medicalcare costs, medication accounted for a major proportion which was52.01%.6.The rank sum test showed that hospitalization duration, model of payment, diseasecondition and diagnosis and treatment quality were correlative factors of medicalexpenses. Results from multiple regression linear analysis indicated that hospitalizationduration was the top influencing factor for inpatient costs.Conclusion1.In the recent10years,the general medical treatment and diagnosis levelof the certain tertiary hospital in Anhui Province improved greatly.Different kinds ofaccidents such as traffic accident,accidental injury,work-related injury and so on arebecoming the important problems of public health.The non-contagious chronic diseasesincluding cerebral infarction,bronchitis and diabetes which closely associated with thelife are increasing gradually.The common diseases are still the primary task of thehospital in the long term.2.In order to control inpatient expenditure, firstly,Wet ought to reduce drug cost andhospitalization duration.
