Analysis On The Influencing Factors Of Hospitalization Expenses Of Coronary Heart Disease Patients In A Top Three Hospital In Henan Province

ObjectiveTo study the hospitalization expenses of coronary heart disease patients in a top three hospital in Henan Province from 2016 to 2018,analyze the composition of hospitalization expenses,and explore the influence of various related factors on hospitalization expenses,so as to provide basis for controlling the excessive growth of hospitalization expenses of coronary heart disease.MethodsA total of 3329 inpatient records of coronary heart disease patients in a top three hospital in Henan Province from 2016 to 2018 were collected,a database was established by Excel,nonparametric test and multiple linear regression analysis were conducted by spss21.0 software,and the factors affecting the inpatient cost of coronary heart disease were analyzed.Results:1.The average age of patients with coronary heart disease is 65.22 years old,and 2432 patients aged 50-80 years old,accounting for 73.1%;the average age of women is 67.93 years old,higher than that of men,62.79 years old;the number of men is more than that of women,but the difference is not significant.52.7%and 47.3%respectively;the number of freelancers is 1578,accounting for 47.4%;the number of patients hospitalized less than 10 days is 1778,accounting for 5%3.4%;416 patients(12.5%);most of the patients were critical patients(83.9%).2.There was a significant difference in the hospitalization expenses of the patients with coronary heart disease of different ages(?2=90.411,P<0.001),and the hospitalization expenses of the patients aged 60-70 were the highest;the hospitalization expenses of the patients with different hospitalization days were statistically significant(?2=1464.71,P<0.001),and the hospitalization expenses were positively correlated with the number of days in Hospital;the hospitalization expenses of the patients with different years were statistically significant(?2=127.919,P<0.001),in 2017,the average hospitalization cost of patients was the highest;the difference of daily hospitalization cost of patients with different hospitalization days was statistically significant(?=94.389,P<0.001),the daily hospitalization cost of patients decreased with the increase of hospitalization days;the average hospitalization cost of patients who had transferred to another department was higher than that of patients who had not transferred to another department;the average hospitalization cost of patients who had transfused blood was higher than that of patients who had not transfused blood;the average hospitalization cost of patients who had undergone surgery was higher than that of patients who had not transfused blood The average hospitalization cost was higher than that without operation.3.The average daily hospitalization expenses of patients increased from 1231.7 yuan in 2016 to 3215.0 yuan in 2018,with a large increase;among the total hospitalization expenses,the treatment expenses accounted for the highest proportion,ranging from 17.5%to 26.2%;the next was the examination expenses,ranging from 22.1%to 24.5%,showing a downward trend year by year,with the treatment expenses and examination expenses accounting for 40%to 50%of the total hospitalization expenses.4.The results of multivariate analysis showed that the main factors affecting the hospitalization cost were the length of stay,average daily hospitalization cost,age,whether to transfer to another department,whether to transfuse blood,whether to operate or not,and the year of discharge.Conclusion1.The main factors that affect the hospitalization cost of coronary heart disease are hospitalization days,average daily hospitalization cost,age,whether to transfer to another department,whether to transfusion,whether to operate,and the year of discharge.Early diagnosis and treatment of coronary heart disease is of great significance to reduce the cost of hospitalization and improve the prognosis.2.It is an effective measure to control the unreasonable increase of hospitalization expenses for patients with coronary heart disease,to reduce the proportion of expenses for drugs and examinations,and to increase the proportion of expenses for technical services of medical staff.3.The coronary heart disease is included in the clinical pathway management,and the unreasonable increase of hospitalization expenses is effectively controlled through standardized diagnosis and treatment process.
